植入型心律转复除颤器患者放电体验的质性研究

The shock experience in patients with implantable cardioverter defibrillator: a qualitative research

目的描述植入型心律转复除颤器(Implantable Cardioverter Defibrillator,ICD)患者经历放电的真实体验。方法采用现象学质性研究法,深入访谈21例经历ICD放电的患者,并采用Colaizzi 7步分析法分析资料。结果通过分析得出6个主题:放电前有心血管系统症状;感知放电的严重性;放电后采取行动各异;多次放电后总结经验自我应对;设备适应性个体差异大;面临多重挑战。结论 ICD放电患者面临心理压力及多重挑战,加强术前宣教、心理疏导、增强患者安全意识和建立专业化的安装后服务体系等,是未来ICD专业化服务发展的趋势。

Objective To describe the shock experience in patients with implantabie cardioverter defibrillator （ICD）. Methods The phenomenological methodology was used in this study. Twenty-one patients with experience of ICD shocks were interviewed. Data were analyzed using Colaizzi＇s seven-step analysis method. Results Six themes were extracted,including cardiovascular system symptoms before shock,perceived harms of shock,various reac- tions after shock,strategies to cope with shock,different adaptability and facing with multiple challenges. Conclu- sion Patients with experience of ICD shocks suffer from psychological pressure and multiple challenges. It is sug- gested to strengthen preoperative education,psychological counseling and professional follow-up service for patients with ICD.
