多环节质量管控在减少PIVAS退药中的应用实践

Application Practice of Multi-link Quality Management and Control for Reducing the Occurrence of Drug Returning in PIVAS

目的:加强静脉用药配置中心(PIVAS)多个工作环节的质量管控,减少退药现象的发生。方法:综合分析我院PIVAS产生退药的原因后,着重从PIVAS内部工作各环节出发,采取相应措施实施质量管控。通过比较自2013年10月开始实施管控后至2014年4月的工作量与退药百分比的变化来评价相关措施的成效。结果与结论:我院PIVAS采取了提高医嘱审核的效率与正确性、调整输液标签打印时间、改进退药模式及加强药品的管理等多方面措施,退药的百分比例由4.51%降到1.50%,呈现明显的下降趋势。这表明所采取的措施能显著减少退药情况的发生,促进PIVAS质量管理水平的提高。

OBJECTIVE： To strengthen the multi-link quality management and control in Pharmacy intravenous admixture ser- vice （PIVAS） so as to reduce the occurrence of drug returning. METHODS： After the reasons for drug returning in the PIVAS of our hospital were analyzed, with priority given to the work links in the PIVAS, measures were taken to perform quality manage- ment and control. The effects of the measures were evaluated by comparing the workload and the percentages of returned drugs dur- ing Oct. 2013 when the above-mentioned management began to be performed and Apr. 2014. RESULTS ＆ CONCLUSIONS： The PIVAS of our hospital took measures to increase the efficiency and accuracy of prescription review, adjust the print time of infusion labels, improve the mode of drug returning and strengthen drug management, and then the percentage of drug returning decreased from 4.51% to 1.50%, showing an obvious downward trend, which indicated that intervention measures can significantly reduce the occurrence of drug returning and improve the quality management of the PIVAS.
