俯卧位通气的呼吸力学变化趋势 被引量:6

摘要 俯卧位通气作为 ARDS 的治疗手段之一,已被证明可以改善氧合、减轻呼吸机相关性肺损伤、提高患者生存率,其中的机制包括俯卧位时呼吸力学的改善。通过对俯卧位通气时呼吸系统的顺应性、胸腹腔压力、肺应力应变、流速、阻力、呼吸功等呼吸力学方面的变化趋势分析发现,对于重度 ARDS患者,俯卧位通气能获得对患者有益的呼吸力学变化,但俯卧位通气的时机等问题仍需进一步探讨。 Prone position ventilation,as a treatment of acute respiratory distress syndrome,can decrease ventilator-induced lung injury,improve oxygenation and survival rate.The mechanism includes the improvement of the respiratory mechanics.The beneficial respiratory mechanics changes can be found in severe acute respiratory distress syndrome patients in prone position by analysing the respiratory mechanics changes such as compliance of the respiratory system,pressure of the thorax and abdomen,lung stress and strain,flow,resistance and work of breathing in prone position.But many problems still need to be explored,such as the time to implement prone postion.
