慢性心力衰竭临床常见证型与心脏超声结果相关性研究

Correlation of Chronic Heart Failure Clinical Common Syndromes and Cardiac Ultrasound Results

目的:探讨慢性心力衰竭(CHF)常见的中医证型与心脏超声检验所得结果的相关性,总结CHF超声影像的规律性。方法:对选取的106例CHF患者,进行心脏彩超检查,通过测量相关数据,得出心脏指数(CI)、左室质量指数(LVMI)、左室射血分数(LVEF)等相关指标,并利用医学统计学方法分析、归纳,总结中医证型与其检测结果之间的相关性。结果:心肺气虚证组病程最短,与其他各组相比均有显著差异(P<0.05),多在疾病的初始阶段出现,心功能多在Ⅱ级,少有Ⅲ级,阳虚水泛证多为疾病的严重阶段,平均病程最长,与其余各证相比均有显著差异(P<0.05)。心功能则以Ⅳ级多见。心肺气虚组的左室射血分数最高,随着CHF病情的发展,中医证型的不同,LVEF值逐渐减小。心脏指数CI值在心肺气虚证中最高,与其它4组相比存在显著性差异(P<0.05),有统计学意义。在阳虚水泛证中最低,而左室质量指数LVMI值的变化趋势与心脏指数CI值变化趋势正好相反,在心肺气虚证中值最小,在阳虚水泛证中值最大,与其它证型相比均存在显著性差异(P<0.05),有统计学意义。结论:心脏彩超检查得出的有关心脏结构、功能改变的相关理化指标与慢性心力衰竭常见中医证型存在一定的相关性,心脏彩超检查为CHF的中医辨证的客观化提供一定的参考依据。

Objective ：To investigate the correlation between the results and the chronic heart failure （CHF） TCM syn- drome type of cardiac ultrasonic inspection , summarize the CHF ultrasound image. Methods ： 106 cases of CHF patients were selected. By heart color ultrasound examination, the relevant data was obtained by measuring, cardiac index （ CI）, left ventricular mass index （ LVMI）, left ventrieular ejection fraction （LVEF） and other related indicators, and the use of medical statistics methods were used for analysis,induction,the correlation between TCM syndrome type and test the re- sults. Results ： The heart lung qi deficiency syndrome group had the shortest duration, compared with other groups had sig- nificant difference （ P 〈 0.05 ）, and more appeared in the initial stage of the disease, cardiac function in grade II, few in grade III, Yang deficiency and water permit more severe stages was the disease, the average duration of the longest, com- pared with the rest it had no significant difference （ P 〈 O. 05 ）. Heart function class IV was more. Left ventricular ejection fraction of lung qi deficiency group was the highest, with the development of CHF disease, TCM syndromes in different LVEF value decreased gradually. Cardiac index CI value was the highest in heart and lung qi deficiency syndrome, com- pared with the other 4 groups had significant less difference （ P 〈 0. 05 ）, there was statistical significance. The lowest in the edema syndrome due to Yang deficiency,the change trend of CI and cardiac index and left ventricular mass index LV- MI value change trend on the contrary, the smallest in the heart and lung qi deficiency syndrome in the median,maximum edema syndrome due to Yang deficiency syndromes and other than that of the median, and there was significant difference （P 〈0.05 ） , and there was statistical significance. Conclusion：The most common TCM syndromes of relevant physico- chemical indexes related to the change of cardiac structure and function echocardiography derived with chronic heart fail- ure had certain relevance, providing the certain reference objective echocardiography for TCM syndrome differentiation of CHF.
