一期、二期交锁髓内钉固定治疗GustiloⅡ型开放性胫骨骨折疗效对比研究 被引量:2

摘要 目的比较一期与二期交锁髓内钉内固定治疗GustiloⅡ型开放性胫骨骨折的疗效差异。方法回顾性分析2010年6月至2013年6月揭阳市人民医院收治的54例GustiloⅡ型开放性胫骨骨折患者的临床资料,其中32例行一期清创、交锁髓内钉内固定(一期内固定组),22例行一期清创、二期交锁髓内钉固定(二期内固定组)。观察两组患者Johner-Wruhs评分优良率、骨折愈合时间、住院时间、治疗费用及并发症等方面的差异。结果两组在Johner-Wruhs评分优良率、骨折愈合时间、伤口感染率方面差异无统计学意义(P>0.05);一期内固定组在伤口一期愈合率、住院时间及治疗费用方面优于二期内固定组,差异均有统计学意义(P<0.05)。结论一期、二期交锁髓内钉内固定治疗GustiloⅡ型开放性胫骨骨折安全有效,均能取得满意疗效;但与二期内固定相比,一期内固定治疗更能促进伤口愈合,缩短住院时间,费用更加低廉。 Objective To compare clinical efficacy between one-stage and two-stage intramedullary interlocking nail fixation for treatment of Gustilo grade Ⅱ open tibial fractures. Methods Data of 54 patients of Gustilo grade Ⅱ open tibial fractures treated in Jieyang People's Hospital from June 2010 to June 2013, were analyzed retrospectively. They were divided into 2 groups, 32 patients in one-stage fixation group were treated by one-stage debridement and intramedullary interlocking nail fixation, 22 patients in two-stage fixation group were treated by the first-stage debridement and the second-stage intramedullary interlocking nail fixation. Excellent and good rate of Johner-Wruhs scoring, fracture healing time, hospitalization time, treatment costs and complications&nbsp;in two groups were observed. Results Differences of excellent and good rate of Johner-Wruhs scoring, fracture healing time and incidence of infection had no statistical differences between two groups (P 〉0.05); While wound healing rate at one-stage, hospitalization time and treatment costs in one-stage fixation group were better than those in two-stage fixation group, there were statistical differences between two groups (P 〈0.05). Conclusions Being safe and effective, one-stage or two-stage intramedullary interlocking nail fixation for Gustilo grade Ⅱ open tibial fractures could achieve similar efficacy, but compared with two-stage fixation, one-stage fixation could enhance wound healing better, shorten hospitalization time and decrease treatment costs.
