前交叉韧带断裂患者步行时足底压力时间特征 被引量:13

Temporal Characteristics of Plantar Pressure in Patients with Anterior Cruciate Ligament Rupture during Walking

摘要 目的:探讨前交叉韧带(anterior cruciate ligament,ACL)断裂患者步行时足底压力时间特征,为康复评定和运动训练提供参考依据。方法:应用footscan R足底压力测量系统,对64例单侧ACL断裂患者的动态足底压力进行对照研究。分析ACL断裂侧、未伤侧步行时支撑相时间参数(足-地接触时间及其足跟触地阶段、前足触地阶段、全足支撑阶段和前足蹬离阶段的百分比)的5次重测信度及其双侧差异。结果:上述各参数5次重测信度的组内相关系数均大于0.79。ACL断裂侧、未伤侧的足-地接触时间分别为662ms、668 ms(t=2.967,P=0.004),足跟触地阶段分别为8.15%、8.79%(t=2.940,P=0.005),全足支撑阶段分别为35.33%、39.29%(t=4.66,P=0.000);而前足蹬离阶段分别为46.62%、42.66%(t=-4.660,P=0.000)。结论:ACL断裂后步行时的动态足底压力时间参数重测信度良好,ACL断裂侧的足-地接触时间、足跟触地阶段和全足支撑阶段显著缩短,而前足蹬离阶段显著延长。 Objective To evaluate the temporal characteristics of plantar pressure in patients with anterior cruciate ligament (ACL) rupture during walking. Methods 64 subjects with symptomatic unilateral ACL rupture were asked to walk on a footscanR systemO at a self-selected speed. The total foot-ground contact duration and the percentages of initial contact phase ,forefoot contact phase ,foot flat phase, and forefoot push off phase relative to the stance phase were calculated. The reliability of parameters was analyzed with intraclass correlation coefficient(ICC). In addition,the differences in the parameters between ACL rupture side and the healthy contralateral side were compared. Results Greater than 0.79 of ICC was observed in all temporal variables during walking. Shorter total foot-ground contact duration, initial contact phase and foot flat phase, and longer forefoot push off phase during walking were found in ACL-injured side as compared with the healthy contralateral side. Conclusions The temporal plantar pressure variables during walking can detect the ACL rupture reliablely.
