孕晚期子宫下段全层厚度检测对瘢痕子宫再次妊娠分娩方式选择的影响

Impact of lower uterine segment full thickness detection of third trimester for delivery mode selection of uterine scar pregnancy again

目的探讨孕晚期子宫下段全层厚度检测对瘢痕子宫再次妊娠分娩方式选择的指导意义。方法选择本院2012年2月-2013年10月接收的瘢痕子宫产妇200例。按孕晚期子宫下段全层厚度分成两组,各100例,研究组子宫下段全层厚度≥3 mm,对照组子宫下段全层厚度〈3 mm,对两组产妇的产后情况进行比较。结果研究组产妇的自然分娩率明显高于对照组,再次剖宫产中研究组术中及术后出血量、血红蛋白、不全子宫破裂、子宫切口愈合及子宫收缩乏力等均明显优于对照组,差异有统计学意义（P〈0.05）;两组新生儿窒息率比较,差异无统计学意义（P〉0.05）。结论孕晚期子宫下段全层厚度检测对瘢痕子宫再次妊娠分娩方式的选择具有重要指导意义,可降低母婴并发症的发生率,值得推广及应用。

Objective To investigate the guidance significance of lower uterine segment full thickness detection of the third trimester for delivery mode selection of uterine scar pregnancy again. Methods 200 cases of the maternal uterine scar in our hospital from February 2012 to October 2013 were selected,and were divided into two groups according to third trimester full thickness of the lower uterine segment,100 patients in each group,study group full thickness of the lower uterine segment thickness were≥3 mm,control group full thickness of the lower uterine segment thickness were〈3 mm, the postpartum between two groups of puerpera was compared respectively. Results The natural birth rate of study group were significantly higher than that in control group,the intraoperative and postoperative bleeding,hemoglobin,in-complete uterine rupture,uterine incision healing and poor uterine inertia of study group puerpera was significantly bet-ter than that of control group,with significant difference（P〈0.05）;The rate of neonatal asphyxia betwwen two groups＇da-ta was no significant difference （P〉0.05）. Conclusion The Lower uterine segment full thickness detection of third trimester has an important guiding significance for delivery mode selection of uterine scar pregnancy again,which can reduce the incidence of maternal complications,and it’s worthy of promotion and application.
