二维彩色血流汇聚法定量评估二尖瓣偏心性反流程度的价值

摘要 目的 探讨二维彩色血流汇聚法(2D-PISA)定量评价二尖瓣偏心性反流程度的价值.方法 选择单纯性二尖瓣偏心性反流患者31例,用2D-PISA法定量评价二尖瓣反流相关参数,以心脏磁共振(CMR)结果为标准,分析两者的相关性,建立线性回归方程.结果 2D-PISA法有效反流口面积(EROA2D-PSA)为(0.19±0.07)cm2,反流容积(RV2D-PSA)为(28.74±9.51)ml,反流分数(RF2D-PISA)为(24.28±7.95)%;CMR法反流容积(RVCMR)中位数为24.0 ml,四分位数间距为15.75 ml,反流分数(RFCMR)为(35.38±14.89)%.各参数2D-PISA法与CMR法间存在良好的相关性,相关系数均>0.60,P均<0.01;EROA2D-mA&RFCMR间直线回归方程为Y=7.24+149.011 X,RF2D-PISA&RFCMR间直线回归方程为Y=7.216+1.16 X.结论 2D-PISA法可较准确地定量评价二尖瓣偏心性反流程度,有良好的应用前景. Objective To evaluate the value of two-dimension proximal isovelocity surface area (2D- PISA) in quantitative diagnosis of eccentric mitral regurgitation (EMR). Methods The parameter of EMR was collected with 2D-PISA in 31 patients. The results were compared with corresponding measurements obtained by cardiac magnetic resonance imaging (CMR), and analyzed their correlation and regression based on the CMR method as a "reference standard". Results The mean EROA2D_PSA was (0.19± 0.07)cm2, mean RV2D_PSA was (28.74± 9.51)ml and mean RF2D-PSA was (24.28± 7.95)%. The median RVCNR was 24.00 ml, the interquartile range was 15.75 ml, the mean RFCMR was (35.38+14.89)%. There was good correlation between the EMR parameter ob- tained by 2D-PISA method and CMR method (r〉0.60, P〈0.01 ). The regression equations between EROA2D_PSA & RFCMR was Y=7.24+149.011 X, and between RF2D_PSA & RFCMR was Y=7.216+1.16 X. Conclusion 2D-PISA is a accurate method to quantitative diagnosis of EMR, it would be widely used in clinic.
