纤维支气管镜吸痰治疗重症肺部感染患者的临床疗效研究

Observation on clinical efficacy of sputum suction by fiber bronchoscopy for severe pulmonary infections

目的观察临床采用纤维支气管镜吸痰技术治疗重症肺部感染患者的效果,为临床首选治疗方式提供参考依据。方法选择呼吸内科2009年1月-2012年12月收治的56例重症肺部感染患者,将其按照治疗方式不同分为对照组和治疗组,对照组28例采用临床常规抗炎抗感染治疗,治疗组28例患者在对照组治疗基础上应用纤维支气管镜吸痰技术进行治疗,对比分析两组患者体温恢复和临床症状改善、血像正常及住院时间和临床疗效与不良反应发生率。结果治疗组临床症状改善、体温恢复正常、血像正常及住院时间分别为(13.95±4.38)、(5.93±2.64)、(11.75±3.86)d及(16.87±4.52)d,明显小于对照组的(23.18±10.23)、(14.75±5.67)、(18.76±4.32)d及(22.56±6.35)d,差异有统计学意义(P<0.05);治疗总有效率治疗组为96.43%,对照组为64.29%,治疗组明显高于对照组,差异有统计学意义(P<0.01);两组患者均未发生不良反应。结论采用纤维支气管镜吸痰技术治疗重症肺部感染,疗效确切、治愈率较高。

OBJECTIVE To observe the efficacy of sputum suction by fiber bronchoscopy used in treating patients with severe pulmonary infections ,so as to provide reference for clinical treatment of first choice .METHODS The 56 patients with severe pulmonary infections admitted in the Department of Respiratory Medicine from Jan .2009 to Dec .2012 were enrolled as subjects of the study .The patients were divided into the control group and the treat‐ment group according to different treatment modalities .The 28 patients in the control group were applied with clinically routine anti‐inflammatory anti‐infective therapy ,and the 28 patients in the treatment group were treated with sputum suction by fiber bronchoscopy in addition to the therapy used in the control group .The time to recov‐ery of body temperature ,improvement of clinical symptoms ,normal hemogram ,and the hospital stay as well as the clinical efficacy and the adverse events were analyzed and compared between the two groups of patients . RESULTS The time to improvement of clinical symptoms ,recovery of body temperature ,normal hemogram and the hospital stay in the treatment group was （13 .95 ± 4 .38） days ,（5 .93 ± 2 .64） days ,（11 .75 ± 3 .86） days and （16 .87 ± 4 .52） days ,significantly lower than （23 .18 ± 10 .23） days ,（14 .75 ± 5 .67） days ,（18 .76 ± 4 .32） days and （22 .56 ± 6 .35） days in the control group ,the differences were significant （P〈0 .05） .The total effective rate was 96 .43% in the treatment group ,significantly higher than 64 .29% in the control group （P〈 0 .01） .No adverse reactions occurred in either group .CONCLUSION Sputum suction by fiber bronchoscopy can achieve exact efficacy ,with the cure rate high .
