医护人员手卫生依从性与老年患者感染相关性研究

Correlation of hand hygiene compliance of medical staff and infections in elderly patients

目的探讨医护人员手卫生依从性与老年患者感染的相关性,为干预和减少临床感染提供依据。方法选取2013年2-12月住院治疗的200例老年患者为研究对象,同时将56名医护人员纳入研究,对所有入选医护人员进行相关知识的培训,并对医护人员的手卫生依从性进行分析,分别统计感染和未感染患者在住院治疗过程中,接受医护人员接触性治疗护理操作次数及相应的医护人员洗手次数,计算洗手率。结果 200例老年患者有18例发生接触性感染,感染率9.00%;感染患者相应的医护人员在实施接触性操作前的洗手率及洗手后手卫生达标率,均明显低于未感染患者,差异均有统计学意义(P<0.05);感染患者分离出病原菌种类、数量与手卫生不达标患者检出病原菌比较,差异无统计学意义。结论老年患者住院发生感染和医护人员手卫生依从性直接相关,应加强医护人员手卫生管理,以减少接触性感染,有效降低医院感染率。

OBJECTIVE To investigate the correlation of hand hygiene compliance of medical staff and infections in elderly patients so as to provide a basis for intervention and reduce of clinical infections .METHODS Totally 200 elderly hospitalized patients during Feb .to Dec .2013 were enrolled as well as 56 medical staff .All the enrolled medical staff were given training of related knowledge and the hand hygiene compliance of the medical staff was analyzed .The times of contact nursing that received by patients with or without infection were statistically summarized and the corresponding hand‐washing times of medical staff were also summarized to calculate the hand‐washing rate .RESULTS Totally 18 out of the 200 elderly patients developed contact infection ,with the infection rate of 9 .00% .The hand‐washing rate and the pass rate of hand hygiene after hand washing in the medical staff treating infected patients were significantly lower than those treating uninfected patients （ P〈 0 .05 ） . The difference was not significant in comparison of the types and amount of pathogens between patients with infections and patients with substandard hand hygiene .CONCLUSION The occurrence of infections during hospitalization in elderly patients is directly related to hand hygiene compliance of medical staff . Hand hygiene management of medical staff should be strengthened to reduce contact infections and effectively reduce hospital infection rates .
