新出现的右束支阻滞对冠状动脉搭桥术后急性心肌缺血的诊断意义

Significance of newly occurred right bundle branch block in acute myocardial ischemia after coronary artery bypass grafting surgery

目的探讨新出现的右束支阻滞对冠状动脉搭桥术后急性心肌损伤的诊断意义。方法回顾性分析241例接受冠状动脉旁路移植手术的冠心痛患者的心电图及心脏生物标志物资料。结果新出现的右束支阻滞组患者肌钙蛋白I峰值显著高于对照组。结论CABG围术期新发生右束支阻滞对围术期急性心肌缺血具有显著的诊断意义。

Objective To investigate the significance of newly occurred right bundle branch block in periprocedural myocardial infarction （PMI） after coronary artery bypass grafting surgery （CABG）. Methods A retrospective review of 259 patients with CABG was made. Electrocardiography records and myocardial markers were compared among patients with different outcomes. Results Cardiac Troponin I level was significantly higher in patients with nRBBB than those in patients without cardiac event. Conclusions Newly occurred RBBB after CABG surgery is significant in diagnosis of new, perioperative ischemic events.
