无创正压通气治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭疗效观察

Efficacy of noninvasive positive pressure ventilation on chronic obstructive pulmonary disease complicated with type Ⅱ respiratory failure

目的探讨无创正压通气（NIPPV）治疗慢性阻塞性肺疾病（COPD）合并Ⅱ型呼吸衰竭的临床疗效。方法选择解放军159中心医院2012年1月至2014年3月收治的79例COPD合并Ⅱ型呼吸衰竭患者，根据随机的原则将患者分为治疗组40例与对照组39例，患者均给予常规治疗，治疗组在其基础上给予NIPPV呼吸机。结果两组患者心率（HR）、呼吸频率（RR）、氧分压（PaO2）、二氧化碳分压（PaCO2）、脉搏氧饱和度（SpO2）比较差异均有统计学意义（P〈0．05）。结论NIPPV是一种有效的治疗COPD合并Ⅱ型呼吸衰竭并改善其临床症状的方式，具有一定的临床应用价值。

Objective To investigate the clinical efficacy of noninvasive positive pressure venti- lation on chronic obstructive pulmonary disease complicated with type Ⅱ respiratory failure. Methods From January 2012 to March 2014, 79 COPD patients complicated with type Ⅱ respiratory failure were se- lected, according to the random principle, the patients were divided into treatment group and control group, the patients were given conventional treatment, and on the basis, the patients were given the NIPPV ventilator in treatment group. Results There were significant differences in the HR, RR, PaO2, PaCO2, SpO2 between the two groups （P 〈 0. 05 ）. Conclusions NIPPV is an effective treatment for COPD with type II respiratory failure, which can improve the clinical symptoms, it has certain clinical value.
