钼靶X线摄影联合彩色多普勒超声检查对乳腺癌的临床诊断价值 被引量:15

摘要 目的探讨钼靶X线摄影联合彩色多普勒超声检查对乳腺癌的临床诊断价值。方法选取2011年1月至2013年12月本院门诊查体拟诊的120例乳腺癌患者,分别行彩色多普勒超声及钼靶X线摄影检查,并与患者术后病理结果进行对照,评价彩色多普勒超声、钼靶X线摄影及两种方法联合检查对乳腺癌的诊断价值。结果彩色多普勒超声诊断乳腺癌阳性预测值为95.9%,阴性预测值为31.8%,敏感度为84.2%,特异度为66.3%,准确度为81.2%;钼靶X线摄影诊断乳腺癌阳性预测值为95.1%,阴性预测值为35.2%,敏感度为89.9%,特异度为65.2%,准确度为82.9%;两种方法联合检查诊断乳腺癌阳性预测值为98.1%,阴性预测值为73.3%,敏感度为96.3%,特异度为84.6%,准确度为92.4%,两种方法联合检查阳性预测值、阴性预测值、敏感度、特异度及准确度均高于单独检测,差异具有显著性(P<0.05)。结论彩色多普勒超声联合钼靶X线摄影用于乳腺癌的检测能够相互弥补因检测原理差异造成的不足,提高乳腺癌的检测效能及诊断效果。 Objective To investigate the clinical value of color doppler ultrasound combined with mamm-ography X-ray photography on dignosis of breast cancer. Method Selected from January 2011 to December 2013 in our hospital outpatient examination of 120 cases diagnosed breast cancer patients, and were detected with color doppler ultrasound and mammography X-ray photography, and diagnosis were contrasted with postoperative pathology. Evaluated of color doppler ultrasound, mammography X-ray photography and the joint inspection two methods of diagnosis on breast cancer. Result Positive predictive value was 95.9%, negative predictive value was 31.8%, sensitivity was 84.2%, speciifcity was 66.3%, accuracy was 81.2%with detected of color doppler ultrasound on breast cancer;positive predictive value was 95.1%, negative predictive value was 31.8%, sensitivity was 89.9%, speciifcity was 65.2%, accuracy was 82.9%, with detected of mammography X-ray photography on breast cancer; positive predictive value was 98.1%, negative predictive value was 73.3%, sensitivity was 96.3%, specificity was 84.6%, accuracy was 92.4%with detected of mammography X-ray photography combined with color doppler ultrasound on breast cancer. Positive predictive value, negative predictive value, sensitivity, speciifcity and accuracy were higher in detected of mammography X-ray photography combined with color doppler ultrasound than single detection, the differences were significant (P〈0.05). Conclusion Color doppler ultrasound combined with mammography X-ray photography applied to the detection of breast cancer can make up for deifciencies due to the detection principle of mutual differences caused by the effect of improving the detection and diagnosis of breast cancer efifcacy.
