妊娠期高血压疾病患者妊娠晚期甲状腺功能筛查结果 被引量:9

摘要 目的 探讨妊娠期高血压疾病患者妊娠晚期甲状腺功能异常的患病情况. 方法 采用病例对照研究,以2012年6月至2013年4月在首都医科大学附属北京友谊医院产科分娩的95例妊娠期高血压疾病患者为高血压组(其中重度子痫前期27例,轻度子痫前期16例,妊娠期高血压19例,慢性高血压20例,慢性高血压并发子痫前期13例),以同期在本院产前检查并分娩的808例无内科合并症、产科并发症和甲状腺疾病高危因素的孕妇为对照组.高血压组在妊娠晚期入院时采集空腹血,对照组在妊娠34周后常规采集空腹血,分离血清后,采用化学发光免疫法测定促甲状腺激素(thyroid stimulating hormone,TSH)、游离甲状腺素(free thyroxine,FT4)和甲状腺过氧化物酶抗体(thyroid peroxidase antibody,TPOAb)水平,比较2组孕妇TSH、FT4和TPOAb中位数值及甲状腺功能异常的患病情况.分别采用独立样本t检验、Mann-Whitney U检验、KruskalWallis秩和检验或Fisher精确概率法对数据进行统计学分析. 结果 高血压组和对照组TSH、FT4、TPOAb水平[M(最小值~最大值)]分别为2.05 (0.08~8.01)与2.11(0.01~9.88) mU/L、7.59(4.38~15.44)与7.85(3.86~34.36) pmol/L以及44.00(7.60~1 300.00)与40.90(1.82~1 300.00) mU/L,2组差异均无统计学意义(P值均>0.05).高血压组甲状腺功能异常总患病率为11.6%(11/95),TPOAb阳性率为9.5%(9/95);对照组甲状腺功能异常总患病率为8.3%(67/808),TPOAb阳性率为6.6%(53/808).2组甲状腺功能异常总患病率和TPOAb阳性率比较,差异均有统计学意义(P值均< 0.05). 结论 随妊娠期高血压疾病的严重程度增加,甲状腺功能异常患病率增高,重度子痫前期患者甲状腺功能异常患病率最高,并以亚临床甲减最常见。
