奥氮平 利培酮 阿立哌唑对康复期精神分裂症患者记忆功能的影响

The effects of olanzapine, risperidone, and aripiprazole on memory function in convalescence schizophrenia

目的：探讨奥氮平、利培酮、阿立哌唑对康复期精神分裂症患者记忆功能的影响。方法将90例康复期精神分裂症患者随机分为3组，每组30例，分别口服奥氮平、利培酮、阿立哌唑治疗，观察12周。于治疗前后采用阳性与阴性症状量表评定精神症状，霍普金斯词汇学习测验‐修订版及韦氏记忆量表‐第3版的空间广度测验评定记忆功能。结果治疗后3组患者阳性与阴性症状量表总分及各因子分均较治疗前显著降低（P＜0．01），同期3组间评分比较差异均无显著性（P＞0．05）。阿立哌唑组霍普金斯词汇学习测验各因子分均较治疗前显著升高（P＜0．05或0．01），奥氮平组和利培酮组的即刻回忆、回忆总数及再认因子分较治疗前显著升高（ P＜0．05或0．01）；阿立哌唑组空间广度测验总分及顺行因子分较治疗前显著升高（P＜0．05），奥氮平组和利培酮组总分及各因子分治疗前后比较差异均无显著性（P＞0．05）。结论奥氮平、利培酮、阿立哌唑均能显著改善康复期精神分裂症患者的各种精神症状及部分认知功能，但阿立哌唑改善患者记忆功能的效果优于奥氮平和利培酮。

Objective To explore the effects of olanzapine ,risperidone ,and aripiprazole on memory func‐tion in convalescence schizophrenia .Methods Ninety convalescence schizophrenics were randomized to three groups of 30 ones each and took orally olanzapine ,risperidone or aripipraole for 12 weeks .Before and after treatment mental symptoms were assessed with the Positive and Negative Syndrome Scale （PAN‐SS） and memory functions with the Hopkins Verbal Learning Test‐Revised （HVLT‐R） and Spatial Span Task of Wechsler Memory Scale‐Third Edition （WMS‐Ⅲ） .Results After treatment the total and each factor scores of 3 groups lowered more significantly compared with pretreatment （P〈0 .01） ,there were no significant differences among three groups （P〉 0 .05） .Compared with pretreatment each factor score of the HVLT‐R heightened more significantly in aripiprazole group （P〈0 .05 or 0 .01） ,so did immediate re‐call ,total recall and recognition in olanzapine and risperidone group （P〈0 .05 or 0 .01）;the total and dig‐its forward score of the WMS‐Ⅲ heightened more significantly in aripiprazole group （P〈 0 .05） ,there were no significant differences in the total and each factor scores between pre‐ and post‐treatment in olanzapine and risperidone group （P〉 0 .05） .Conclusion Olanzapine ,risperidone ,and aripiprazole all could notably improve various kinds of mental symptoms and partial cognitive functions ,but aripiprazole has an advantage in improving memory function over olanzapine and risperidone .
