简化认知行为治疗操作手册的编制与临床应用评价

Establishment and clinical evaluation of Simplified Cognitive Behavioral Therapy manual

目的编写适用于广泛性焦虑(GAD)的简化认知行为治疗(SCBT)操作手册。方法通过文献检索、相关培训和临床实践,针对GAD的症状特征编写SCBT操作手册。28例GAD患者随机分为SCBT组(研究组)和SCBT+药物组(对照组)各14例进行为期8周的治疗,比较治疗前后两组患者自评疗效和误工时间,及16位治疗师的使用评价。结果建立整套适用于GAD的SCBT治疗师操作手册,12次治疗安排有连贯性。治疗后第8周末患者疗效自评研究组高于对照组(P=0.010);两组误工时间均较各自治疗前减少(P<0.01);研究组误工时间低于对照组(P=0.004)。16位治疗师中有12名认为比较容易学习手册中的内容,14名认为对患者的适用性比较高。结论 SCBT对GAD针对性和有效性较强,并具有良好的可操作性。

Objective To establish a Simplify Cognitive Behavioral Therapy （SCBT）manual appropriate for generalized anxiety disorder （GAD）.Methods SCBT manual aiming at the symptom characteristics of GAD was compiled after literature retrieval,related training and clinical practices.28 patients with GAD were randomized into study group （14 cases ）treated with SCBT and control group （14 cases）treated with SCBT and antianxietic for 8 weeks.The evaluate indicators included self -rated efficacy and the downtime at baseline and at the end of the 8-week treatment,as well as the evaluations of 16 therapeutists. Results A set of SCBT manual used by therapeutists was established for GAD,which consisted of 12 coherent sessions.The self-rated efficacy at the endpoint was significantly higher in study group than that in control group （P =0.010）.The downtime at endpoint in both groups decreased significantly when compared with the baseline （P 〈0.01）,and it was significantly lower in study group than that in control group （P =0.004）.12 out of 16 therapeutists reported that SCBT manual was easy to learn,and 14 out of 16 therapeutists reported that SCBT manual had good applicability.Conclusion SCBT has good pertinency and efficacy for GAD,and can be used conveniently.
