摘要
目的探讨磁共振脑表面重建影像墨卡托视图(Mercator View)在显示中央区结构及定位中央沟中的作用。方法 2013年1-12月在我科实施导航手术的52例病例中选择出应用3D T1 MPR序列获取影像数据的患者资料43例。将患者的影像三维数据通过磁共振工作站进行曲面重建得到墨卡托视图。由神经外科和影像科医师分别对该资料进行分析,根据中央沟及其周围结构的影像特点定位中央沟。结果共得到正常的中央区资料73个,有病变的中央区资料13个。正常中央区均能够在墨卡托视图上得到清晰的显示和明确的定位,13例有病变的中央区中92.3%的中央沟能够得到准确的定位,而且不同人员对其定位结果均一致。结论中央区结构可以在单幅脑表面重建墨卡托视图上得到完整的显示,有助于更直观、更准确地判断中央沟位置。
Objective To evaluate the advantage of the brain surface reformatted images in displaying anatomy of the central region. Methods Forty-three patients with a 3D T1 MPR data out of 52 patients that accepted navigation surgery in our department between Jan 2013 and Dec 2013 were recruited. Brain surface reformatted images (BSR) of 86 hemispheres in 43 patients were generated from three-dimensional magnetic resonance imaging data sets using a curved multiplanar reformatting technique. These BSR images were reconstrufted in less than 5 minutes and demonstrated the entire central sulcus with adjacent surface sUuctures in one Mercator view. Images were evaluated by two experienced neuroradiologists and two neurosurgeons to identify the central region according to the presence of the imaging landmarks of the pericentral region. Results Mercator views of 73 hemispheres with normal anatomy and 13 with lesion were evaluated. All the 73 hemispheres with normal anatomy had an unambiguous identification of the central region. As for the 13 hemispheres with central region lesion, an accurate identification of the central sulcus could be achieved in 92.3% by using our strategy to evaluate the Mercator view. Conclusions The central region can identified easily and precisely in a single intuitionistic brain surface reformatted Mercator view. Learn more about the anatomy landmarks of the pericentral region in the Mercator view will greatly improve it.
