腰硬联合麻醉和静脉全身麻醉用于老年患者髋部手术的临床比较

目的为了对比分析在老年病患髋部手术中腰硬联合麻醉与静脉全身麻醉的治疗效果。方法选取2011年9月—2012年9月在该院接受髋部手术的老年病患70人,将其随机分成两组,治疗组与对照组,每组共有病患35人,为治疗组中的病患提供腰硬联合麻醉,为对照组中的病患提供静脉全身麻醉,对比两组病患的治疗效果。结果治疗组中病患术中疼痛率为68.6%(24/35),对照组中病患术中疼痛率为28.6%(10/35),治疗组疼痛率明显高于对照组,差异有统计学意义(P<0.05)。治疗组中病患术中的心率与血压情况明显优于对照组,差异有统计学意义(P<0.05),但是术后12 h,两组病患的心率与血压差异无统计学意义;治疗组中病患术后8 h VAS评分明显优于对照组,差异有统计学意义(P<0.05),但是术后12 h两组病患的VAS评分差异无统计学意义。结论临床治疗老年髋部手术病患时,腰硬联合麻醉与静脉全身麻醉各有利弊,需要根据病患的身体情况选择合适的麻醉方式。

Results Rate of intraop-erative pain patients in treatment group was 68.6% （24/35）, the rate of intraoperative pain patients in the control group was 28.6%（10/35）, pain rate of treatment group was obviously higher than that of control group, P 〈 0.05, statistically significance. the heart rate and blood pressure in patients in the treatment group was better than control group, P〈0.05, statistically significance, but af-ter 12 hours, two groups of patient＇s heart rate and blood pressure has no obvious difference; Patients in treatment group after 8 hours of VAS score was better than control group, P 〈 0.05, statistically significance, but after 12 hours no difference between the two groups of patients VAS score. Conclusion Clinical treatment of senile hip surgery patients, waist hard joint anesthesia with in-travenous general anesthesia each have advantages and disadvantages, need according to the patient＇s physical condition to choose the right means of anesthesia.
