腹腔镜下卵巢良性肿瘤60例临床分析

Clinical analysis of 60 cases of benign ovarian tumor under laparoscopy

目的通过对60例腹腔镜手术下卵巢良性肿瘤患者的临床治疗效果观察,探究腹腔镜手术治疗卵巢良性肿瘤的效果及相关临床因素,并与传统的开腹手术临床效果进行对比。方法回顾性分析2013年1月—2014年5月期间在该院接受治疗的60例卵巢良性肿瘤患者的临床资料,该组60例患者在术前皆经过检查初步确诊为卵巢良性肿瘤,此组患者的腹腔镜手术全都顺利完成,无中转开腹;为对比腹腔镜手术治疗卵巢良性肿瘤的临床治疗效果,另外选取60例同样病理特征的患者作为参照组,参照组采取传统的手术方式进行治疗,对两组的治疗效果进行对比。结果采用腹腔镜手术治疗卵巢良性肿瘤手术时间36-81 min,平均时间为48.68 min,出血量15-245 m L,平均86.95 m L,术中无并发症出现,患者术后8.5 h起床活动,2-17 h排气,平均6.14 h,住院时间3-5 d,平均4.25 d;而采用开腹手术治疗卵巢良性肿瘤手术时间为43-120min,平均时间为75.28 min,出血量55-510 m L,平均151.28 m L,术中无并发症出现,患者24 h后起床活动,7-39 h排气,平均20.12 h,住院时间为5-9 d,平均6.55 d,以上结果表明腹腔镜手术临床效果显著,差异有统计学意义（P〈0.05）,要优于传统的开腹手术方式,可以在临床上推广应用,应用价值高。结论对卵巢良性肿瘤患者采用腹腔镜手术方式具有治疗效果好,患者创伤小,出血量少,患者恢复快,手术简单便捷、时间短,住院时间少,并发症少等优点,与传统的手术方法相比具有很大的优越性,值得在临床上推广应用。

Objective through observation of the effect of clinical treatment of 60 cases of laparoscopic operation patients of benign tumor of ovary, inquiry of laparoscopic operation in treatment of benign ovarian tumor effects and related clinical factors, and com-pared with the clinical effect of traditional open operation. Methods a retrospective analysis of the clinical data of 60 cases of be-nign ovarian tumor from 2013 January to 2014 May in our hospital treated patients, 60 cases in this group are checked before surgery in patients with initial diagnosis as benign ovarian tumor, laparoscopic operation of the patients in this group were all com-pleted successfully, without conversion to laparotomy; for the clinical treatment effect comparison laparoscopic operation in treat-ment of benign ovarian tumor, another 60 cases selected the same pathological features of the patients served as a control group, the reference group to take the traditional operation treatment, compared two groups of treatment effect. Results laparoscopic surgery for benign ovarian tumor operation time of 36-81min is adopted, the average time is 48.68min, the amount of bleeding from 15 to 245 ml, average 86.95ml, no complications during operation, postoperative up activity in patients with 8.5H, 2 - 17h exhaust, average 6.14h, hospitalization for 3-5 days, an average of 4.25 days; and the use of open surgical treatment of benign o-varian tumor operation time was 43 120min, the average time is 75.28min, the amount of bleeding from 55 to 510ml, average 151.28ml, no complications during the surgery, 24 h patients after get up, 7 - 39H exhaust, average 20.12h, the time of hospital-ization for 5-9 days, average 6.55 days, the results above showed that the clinical effect of laparoscopic surgery significantly （ P 〈0.05 with statistical significance）, open surgery is better than the traditional laparotomy group, [given data, description of the two groups were compared with statistical difference] can be widely used in the clinic, the application of high value. Conclusion the laparoscopic approach in patients with benign ovarian tumor with good treatment effect, small trauma, less bleeding, patients re-covered quickly, the operation is simple and convenient, the time is short, less hospitalization time, less complications, compared with the traditional surgical method has great superiority, worth in clinical application.
