腹腔镜下肾上腺结核病灶切除治疗效果观察

目的探讨腹腔镜下肾上腺结核治疗方法及治疗效果。方法对2012年3月—2014年3月期间该院泌尿科收治的60例肾上腺结核疾病患者作回顾分析,将患者分成对照组与观察组各30例,观察组患者进行腹腔镜下结核病灶切除术手术,对照组患者进行常规开腹手术。观察患者的手术时间、术中出血量、术后恢复时间、住院时间、术后出血量、术后感染及术后复发情况。结果观察组30例患者的手术时间为(2.58±3.37)h;术中出血量为(87.84±32.51)m L;术后恢复时间为(28.81±8.37)h;住院时间为(3.23±1.54)d;术后出血量为(70.21±38.37)m L,相比对照组手术时间较短、术中出血量和术后出血量较少、术后恢复时间短,两组对比差异有统计学意义(P<0.05);观察组术后感染例数3例(10%),对照组术后感染9例(30%);术后均无复发;两组手术并发症发生率对比差异有统计学意义(P<0.05)。结论如果术前准备充分、术中处理正确,在腹腔镜下治疗肾上腺结核疾病较为安全、可靠,适合临床推广应用。

Objective To investigate the laparoscopic adrenal tuberculosis treatment methods and experience. Methods From March 2012 to March 2012 hospital urology department admitted during the period of 60 patients with adrenal tuberculosis disease to make retrospective analysis, the patients were divided into control group and observation group of 30 cases, observation group of patients with tuberculosis kitchen under laparoscopic resection surgery, control group patients with conventional laparotomy. Results Observation group of 30 patients with surgery for （2.58±3.37） hours;Intraoperative blood loss for ml （87.84±32.51）;The postoperative recovery time was （28.81＋/-8.37） hours. Length of hospital stay （3.23 ±1.54） days;Postoperative blood loss for ml（70.21±38.37）, compared with the control group shorter operative time, intraoper-ative blood loss, postoperative blood loss, postoperative recovery time, compared the two groups statistically significant difference （P〈0.05）;Observation group cases of postoperative infections in 3 patients （10%）, postoperative infection, 9 cases of control group （30%）. There were no postoperative recurrence; The surgical complication rates compared to similar between the two groups have statistical significance （P 〈 0.05）. Conclusion If the preoperative preparation, intraoperative treatment right fully, in laparoscopic treatment of adrenal tuberculosis disease is relatively safe and reliable, suitable for clinical application.
