微切口和常规切口超声乳化分别联合小梁切除术治疗白内障合并青光眼的疗效对比

Comparison of the Effect Between Micro and Conventional Incision Phacoemulsification Combined with Trabeculectomy on Cataract and Glaucoma

目的比较微切口和常规切口超声乳化分别联合小梁切除术治疗白内障合并青光眼的临床疗效差异。方法选取该院2012年1月—2014年7月收治的82例(87眼)白内障合并青光眼患者,按照随机数字表法均分为对照组(41例,43眼)和观察组(41例,44眼),对照组采用常规切口超声乳化联合小梁切除术治疗,观察组采用微切口超声乳化联合小梁切除术治疗,均随访3个月,比较两组手术前后裸眼视力、散光、眼压的变化和并发症发生率的差异。结果两组患者术后裸眼视力、眼压均明显改善,与术前比较差异有统计学意义(P<0.05);术后3个月,观察组裸眼视力显著优于对照组(P<0.05);两组患者散光程度和眼压比较差异无统计学意义(P>0.05);观察组并发症发生率显著低于对照组(P<0.05)。结论微切口超声乳化联合小梁切除术治疗白内障合并青光眼疗效确切,视力改善明显,并发症少。

Objective To compare the clinical effect between micro and conventional incision phacoemulsification combined with trabeculectomy on cataract and glaucoma. Methods 82 cases （87 eyes） with cataract and glaucoma admitted in Shengjing Hospital of China Medical University from January 2012 to July 2014 were selected and divided into the control group （41 cases, 43 eyes） and the observation group （41 cases, 44 eyes） in accordance with the method of random digits table. The control group was treated by conventional incision phacoemulsification combined with trabeculectomy, while the observation group was treated by micro-in-cision phacoemulsification combined with trabeculectomy. Both groups were followed up for 3 months. And the uncorrected visual acuity, astigmatism, intraocular pressure and incidence of complications were compared between the two groups. Results The un-corrected visual acuity and intraocular pressure of the two groups after surgery were significantly better than those before surgery with statistical significance（P〈0.05）. 3 months after surgery, the uncorrected visual acuity of the observation group was much better than that of the control group （P〈0.05）; astigmatism and intraocular pressure between the two groups had no significant difference （P〉0.05）;the incidence of complications of the observation group was much lower than that of the control group（P〈0.05）. Conclu-sion Micro-incision phacoemulsification combined with trabeculectomy has exact effect on cataract combined with glaucoma. It can significantly improve the visual acuity with fewer complications.
