超声引导下甲状腺结节细针吸取细胞学检查的诊断价值探讨

To Discuss the Ultrasound-guided Fine Needle Aspiration in the Diagnosis of Thyroid Nodule

目的探讨超声引导下甲状腺结节细针吸取细胞学的临床价值。方法选取该院2012年1月—2014年1月收治甲状腺结节患者88例,均给予超声引导下甲状腺结节细针吸取细胞学检查,42例给予手术治疗。观察细针吸取细胞学检查结果及敏感度、特异性、诊断符合率、误诊率。结果经细针吸取细胞学检查,良性80例,恶性6例,可疑恶性2例,未见涂片不满意。结节性甲状腺肿47(53.41%)例,甲状腺乳头癌4(4.55%)例。42例手术治疗与术后病理对照计算得细针吸取细胞学诊断敏感度为75.00%,特异性为95.00%,诊断符合率为93.18%,误诊率为5.00%。其余46例随访1年间,均未见失访及转恶性变者。结论超声引导下细针吸取细胞学检查可有效诊断甲状腺结节,且操作简单。

Objective To evaluate the efficacy of ultrasound-guided fine-needle aspiration （US-FNA）in the diagnosis of thyroid nodules. Methods 88 patients with thyroid nodes of neck surgery and endocrinology department in our hospital from Jan ,2012-Jan,2014 , they were all given ultrasound-guided fine-needle aspiration （US-FNA）in the diagnosis of thyroid nodules.42 cases were given surgical treatment .observe the results of ultrasound-guided fine-needle aspiration and sensitivity, specificity and di-agnostic coincidence rate, the misdiagnosis rate. Results After ultrasound-guided fine-needle aspiration,80 cases of benign , 6 cases of suspected, 2 cases of fishy, no smear was not satisfied.Benign mainly was thyroid nodules ,47 （53.41%） cases,suspected mainly was thyroid papillary carcinoma, 4 cases （4.55%）.42 cases of surgical treatment and postoperative pathology was calculated by the fine-needle aspiration cycology diagnosis sensitivity of 75.00%, specificity of 95.00%. The diagnostic coincidence rate was 93.18%, the misdiagnosis rate was 5.00%.The rest of the 46 cases were followed up for 1 year, lost of follow-up and turn malig-nant degeneration was not found. Conclusion Ultrasound-guided fine-needle aspiration（US-FNA）in the diagnosis of thyroid nod-ules has features of simple operation, high accuracy, worth clinical promotion.
