健脾益肺汤改善肺脾气虚型COPD无创机械通气患者呼吸肌疲劳的疗效

Clinical Research of Jianpi Yifei Decoction on Treating COPD Patients of Lung-spleen Qi Deficiency Who Need Noninvasive Mechanical Ventilation

目的该研究主要就肺脾气虚型慢性阻塞性肺疾病(COPD)患者行无创机械通气治疗时给予健脾益肺汤后呼吸肌疲劳的改善情况展开分析。方法纳入120例该院2013年12月—2014年12月所收治的慢性阻塞性肺病患者为对象,根据患者的意愿将其均分为两组,对照组给予常规治疗,观察组患者在此基础上给予健脾益肺汤治疗,比较两组患者的血气分析、呼吸机疲劳以及中医症状等指标。结果两组患者在治疗前后的肺功能相关指标、二氧化碳分压降低率、可升高动脉血氧分压差异有统计学意义P<0.05;治疗后两组患者的肺功能相关指标、二氧化碳分压降低率、可升高动脉血氧分压差异无统计学意义,P>0.05;两组患者在治疗后的最大呼气压以及最大吸气压的升高率差异,有统计学意义,P<0.05。结论在对慢性阻塞性肺病患者进行临床治疗时,在无创通气的基础上对其给予健脾益肺汤可以有效改善患者的呼吸机疲劳状况,避免患者在治疗过程中出现呼吸衰竭的情况。

Objective This study mainly focuses on the lung and spleen deficiency syndrome of chronic obstructive pulmonary disease （COPD） to improve the situation to analyze the respiratory muscle fatigue patients with noninvasive mechanical .Methods The patients included 120 cases of chronic obstructive lung disease in our hospital from 2013 December to 2014 December were as the object, according to the wishes of patients which were divided into 2 groups, the control group was given conventional treatment, the observation group were treated on the basis of Jianpi Yifei decoction treatment, blood gas analy and TCM symptoms index.Results The two groups of patients before and after treatment in pulmonary function related indicators, partial pressure of carbon dioxide reduction rate, can increase the arterial oxygen partial pressure differences, with statistical significance,P〈0.05.Conclusion Noninvasive ventilation on the given Jianpi Yifei decoction can effectively improve patient ventilator fatigue status, avoid respiratory failure occurred during the course of treatment in patients with.
