基于胱抑素C的GFR估算公式在亲属活体肾移植术后受体肾功能评价中的应用评估 被引量:1

摘要 目的 :评估与比较4种常见的以血清胱抑素C(cystatin C,Cys C)为基础的肾小球滤过率(glomerular filtration rate,GFR)估算公式在亲属活体肾移植术后受体肾功能评估中的应用。方法:选取39例在本院行亲属活体肾移植且肾功能稳定的肾移植受体,以99m Tc-DTPA肾功能动态显像技术(ECT)测定的GFR(reference GFR,r GFR)作为基准,比较4种以Cys C为基础GFR估算公式(Larsson公式、Rule公式、Hoek公式和Filler公式)在亲属活体肾移植受体肾功能评估中的偏倚、精度和基准试验结果30%及50%的精确度。结果:Larsson公式具有最高的精确度(30%精确度:64.1%;50%精确度:89.7%)和最好的精度[11.2 ml/(min·1.73 m2)],偏倚也仅为8.8 ml/(min·1.73 m2)。结论 :在亲属活体肾移植受者术后长期随访中,以Cys C为基础的Larsson公式可能是评估及监测移植肾GFR的最佳选择。 Objective:To evaluate the performance of 4 cystain C(Cys C)-based glomerular filtration rate(GFR)estimating equations in living related donor transplantation recipients. Methods: We compared the performance of 4 Cys C-based GFR estimating equations (the Larsson equation,the Rule equation,the Hoek equation and the Filler equation)using GFR measurement of 99mTc-DTPA as the reference test in 39 stable renal transplantation populations from living related donor in our hospital. Bias,precision, accuracy within 30% and 50% ranges from the reference method and agreements of each test were compared. Results:The Larsson equation had the least precision [ 11.2 ml/(min- 1.73m2) ] and highest accuracy (64.1% within 30% of the reference). Conclusion: We concluded that the Cys C-based Larsson equation may be the best choice in assessing and monitoring GFR of long-term postoperative renal transplant recipients.
