利普液基细胞学联合免疫细胞化学在细针穿刺诊断甲状腺结节中的价值 被引量:7

摘要 目的 :利用利普液基细胞学(LPT)技术及免疫细胞化学技术,评价半乳糖凝集素-3(Galectin-3)和甲状腺过氧化物酶(thyroid peroxidase,TPO)在甲状腺良恶性结节中的诊断价值。方法 :甲状腺细针穿刺标本80例LPT技术制细胞涂片,用免疫细胞化学方法检测穿刺标本中Galectin-3与TPO的表达情况,并与甲状腺术后组织病理相比较,评价Galectin-3和TPO在甲状腺良恶性病变中的表达率。结果:术前行细针穿刺细胞学(fine needle aspiration cytology,FNAC)检查的80例患者术后病理确诊为甲状腺癌51例(其中甲状腺乳头状癌49例、甲状腺滤泡状腺癌2例),良性病变29例(含慢性淋巴细胞性甲状腺炎2例、滤泡性腺瘤7例、结节性甲状腺肿20例)。免疫组化检测Galectin-3在甲状腺乳头状癌和滤泡性腺癌中明显高表达,分别为93.8%和100%,总阳性率为94%(47/51);TPO在慢性淋巴细胞性甲状腺、结节性甲状腺肿和滤泡性腺瘤中明显高表达,阳性表达率分别为100%、100%和85.7%,总阳性表达率为96.7%(28/29)。结论:液基细胞学技术与免疫细胞化学分析相结合在甲状腺结节FNAC标本诊断中有重要的临床意义,细胞分子标志物Galectin-3和TPO可用于甲状腺良恶性肿瘤的鉴别诊断,提高了甲状腺癌的术前诊断率,值得在临床细胞病理学诊断中推广应用。
