子宫剖宫产部位瘢痕妊娠超声诊断的临床意义

Clinical value of color Doppler ultrasound of the cesarean scar pregnancy diagnosis

目的 :探讨彩色多普勒超声检查在子宫剖宫产部位瘢痕妊娠诊断和治疗中的价值。方法 :117例患者分成孕囊型(72例,其中4例行病灶切除术,68例未行病灶切除术)和混合回声型(45例,其中11例行病灶切除术,34例未行病灶切除术),她们的病史、临床诊治情况、结局及超声声像图进行回顾性分析。结果 :经校正χ2检验,行瘢痕部位病灶切除术的病例中混合回声型高于孕囊型,有明显的统计学意义。结论 :彩色多普勒超声检查在剖宫产部位瘢痕妊娠早期诊断、治疗及随访观察中具有重要意义。

Objective： To explore the application value of color Doppler ultrasound in the diagnosis and treatment of cesarean scar pregnancy. Methods： Based on the ultrasonogram of 117 cases of patients, they could be divided into a gestational sac type with 72 cases, in which 4 cases were treated by local resection and 68 cases were not, and a mixed echo type with 45 cases, in which 11 cases underwent local resection and 34 cases did not） and their medical history, clinical management, follow up result and sonographic characteristics were retrospectively analyzed. Results： The number of cases receiving cesarean section scar resection was more in the mixed echo type than in the gestational sac type. The result showed a highly statistically significance by using an adjusted formula for chi-square test. Conclusion： Color Doppler ultrasound is of benefit to guiding the early diagnosis, treatment and following-up of cesarean scar pregnancy.
