剖宫产同时子宫肌瘤剔除术48例临床分析

Clinical Analysis of 48 Cases of Hysteromyoma Ectomy Conducted during Cesarean Section

目的探讨剖宫产同时行子宫肌瘤剔除术的必要性和可行性。方法选取2010年11月至2013年1月北京市西城区展览路医院行剖宫产同时子宫肌瘤剔除术(联合手术组)48例及单纯剖宫产的患者(单纯剖宫产组)48例。比较两组间手术时间、术中出血量、术后24 h阴道出血量、住院时间及产后42 d子宫复旧情况。结果联合手术组的手术时间显著高于单纯剖宫产组[(58.3±8.8)min比(30.2±4.2)min,P<0.01];两组术中、术后出血量,住院时间和子宫复旧患者比例比较差异无统计学意义(P>0.05)。结论剖宫产同时剔除子宫肌瘤虽然延长了手术时间,但减少了患者再次手术的痛苦,也减少了产后出血量,手术是必要和可行的。

Objectives To study the necessity and feasibility of applying hysteromyoma ectomy during cesarean section.Methods From Nov.2010 to Jan.2013,48 patients underwent cesarean section and myo-mectomy（ combined operation group ） and 48 patients underwent simple cesarean section （ simple cesarean section group） in Zhanlanlu Hospital of Xicheng District were selected as research objects.The operation time,intraoperative bleeding volume,vaginal bleeding volume within 24 h after operation,average duration of hospital stay and condition of uterine involution 42 d after delivery were compared between the two groups. Results The operation time of the combined operation group was significantly higher than that of the simple cesarean section group[（58.3 ±8.8） min vs （30.2 ±4.2） min,P〈0.01];there were no statistically sig-nificant differences of the intraoperative bleeding volume , vaginal bleeding volume within 24 h after opera-tion,average duration of hospital stay and condition of uterine involution 42 d after delivery between the two groups（ P 〉0.05 ）.Conclusion Despite the prolonged operation time, hysteromyoma ectomy during cesarean section can reduce the amount of postpartum hemorrhage and make less impairment to the pregnant patients, it′s necessary and feasible to perform hysteromyoma ectomy during sesarean section.
