肥胖及2型糖尿病患者血清肿瘤坏死因子相关蛋白9水平及二甲双胍对其的影响 被引量:8

摘要 目的探讨肥胖及2型糖尿病(T2DM)患者血清脂肪肪细胞因子C1q/肿瘤坏死因子相关蛋白9(CTRP 9)水平的变化,观察二甲双胍对其的影响。方法 86例新诊断T2DM患者和88例正常糖耐量(NGT)者,按体质指数(BMI)又各自分为正常体重(NW)与肥胖(OB)亚组。采用ELISA法检测血清CTRP9水平,观察二甲双胍对其水平的影响。结果 (1)4组血清CTRP9水平依次为:T2DM-OB组<T2DM-NW组、NGT-OB组<NGT-NW组(P<0.05,P<0.01);(2)NGT-OB组CTRP9与FPG、Hb A1C、HOMA-IR、BMI、TG呈负相关(r=-0.603、-0.598、-0.595、-0.617、-0.512,P<0.05,P<0.01),与TNF-α、SOD、APN呈正相关(r=0.638、0.528、0.512,P<0.05,P<0.01);T2DM组CTRP 9与FPG、Hb A1C、HOMA-IR、BMI、TG、TNF-α呈负相关(r=-0.596、-0.612、-0.586、-0.603、-0.531、-0.598,P<0.05,P<0.01),与APN、SOD呈正相关(r=0.552、0.512,P<0.05,P<0.01);(3)多元逐步回归分析显示HOMA-IR、Hb A1C、BMI是CTRP9的独立影响因素。(4)二甲双胍治疗后CTRP9水平显著上升(P<0.05)。结论 CTRP9可能在肥胖、胰岛素抵抗和T2DM的发生发展中起到了重要作用。二甲双胍通过升高CTRP9水平发挥了减轻胰岛素抵抗、抗炎、抗氧化应激作用。
