下咽癌调强放疗疗效及对喉咽功能的观察

To Observe the Recent Clinical Curative Effect of IMRT and Influence on Hypopharyngeal Function for Hypopharyngeal Carcinoma

目的观察无远处转移下咽癌患者调强适形放射治疗（intensity-modulated radiotherapy，IMRT）的近期疗效，并评价其对患者喉咽功能的影响。方法选择无远处转移下咽癌患者69例，所有患者均行调强适形放射治疗，原发灶及颈部高危淋巴引流区的放疗处方剂量均为66～70 Gy。放疗结束2月后评价近期疗效及喉咽功能。结果疗效达到CR的患者21例（30.43%），PR患者38例（55.07%），SD患者7例（10.14%），PD患者3例（4.35%）。客观缓解率达到85.5%。全组患者中49例喉咽功能良好，占患者总数的71.01%。结论 IMRT治疗无远处转移的下咽癌的近期疗效好，而且能较好地保留患者的喉咽功能。

Objective To study the recent clinical curative effect of intensity-modulated radiotherapy （ IMRT） and the hypopharyngeal function after radiotherapy for hypopharyngeal carcinoma without distant metastasis. Methods69 patients suffered from hypopharyngeal carcinoma without distant metastasis received IMRT. The prescribed dose for gross target volume and high-risk lymphatic drainage area was 66～70 Gy. The recent clinical curative effect of IMRT and the hypopharyngeal function were assessed 2 momth later after radiotherapy. Results The rates of complete remission （CR）, partial remission （PR）, stabilization （SD） and progressive disease（PD） were 30.43%, 55.07%, 10.14% and 4.35% respectively. The objective response rate was 85.5%. There was 49 cases whose functional laryngeal preservation in all the patients and the rate was 71.01%. ConclusionIMRT can improve the recent clinical curative effect and preverse the hypopharyngeal functions for the patients with hypopharyngeal carcinoma without distant metastasis.
