院感质控小组降低中晚期肿瘤患者手术室感染发生率的临床分析

目的:探讨院感质控小组在降低中晚期肿瘤患者院感率方面的价值,以降低中晚期肿瘤患者治疗期间发生感染的几率。方法:将我院中晚期肿瘤患者纳入院感质控小组重点管理范畴,比较实施重点管理前一年(2011年8月-2012年7月)和后一年(2012年8月-2013年7月)我院中晚期肿瘤患者手术室感染发生率、病原菌构成情况、感染部位构成情况、患者治疗预后情况、医患纠纷发生率及患者QOL生活质量改善及满意度等情况,综合评价院感质控小组重点管理对中晚期肿瘤患者易手术室感染率降低中的临床价值。结果:2012年共收治各类中晚期肿瘤患者125例,共发生手术室感染32例,感染率为25.60%;2013年共收治各类中晚期肿瘤患者132例,共发生手术室感染23例,感染率为17.42%;两年间感染率比较,差异具有统计学意义(P<0.05);两年之间感染病例病原菌分析结果显示,在病原菌构成方面比较无统计学意义(P>0.05);感染部位构成比之间的差异存在统计学意义(P<0.05);医患纠纷发生率及患者满意率、患者生活质量评分比较差异具有统计学意义(P<0.05)。结论:手术室感染是中晚期肿瘤患者病情转危及死亡最为重要的因素之一,手术室感染质控小组加强对此类患者的管理,采取针对性地预防感染措施,可以明显降低中晚期肿瘤患者手术室感染率的发生,提高此类患者临床治疗效果,改善患者生活质量,对中晚期肿瘤患者具有重要意义。

Objective：To investigate quality control team at reducing hospital infection in patients with advanced cancer. Methods： Patients with advanced cancer in our hospital into a sense of quality control team focused on hospi- tal management,the implementation of key management compared the previous year （August 2011 -2012 in July） and after one year （ August 2012 -2013 in July month ） in patients with advanced cancer in our hospital. Results：In 2012 of 125 advanced cancer patients infection developed in 32 cases ,the infection rate was 25.60% ,in 2013 of 132 advanced cancer patients, a total of 23 cases of nosocomial infection, infection rate was 17.42% （P 〈 0.05 ）. Pathogen was no significant difference （ P 〉 0.05 ）. Mortality was statistically significant different （ P 〈 0.05 ）. Quality of life scores were statistically significant different （ P 〈 0.05 ）. Conclusion： Hospital infection QC team can strengthen the management of patients, to take specific measures to prevent infection, can significantly reduce nosocomial infection in patients with advanced cancer.
