温针灸配合委中穴拍打放血治疗腰椎间盘突出症43例临床观察

Clinical Observation on 43 Cases of Lumbar Disc Herniation Treated with Warm Acupuncture and Moxibustion Combined with Beating and Bloodletting at Weizhong( BL 40)

目的观察温针灸配合委中穴拍打放血治疗腰椎间盘突出症的临床疗效。方法将86例腰椎间盘突出症患者随机分为治疗组和对照组各43例,治疗组给予温针灸配合委中穴拍打放血,对照组给予单纯温针灸治疗。两组均每日1次,治疗7次为1个疗程,疗程间隔2天,治疗3个疗程后统计疗效。治疗后比较两组临床疗效、疼痛评分,并检测治疗前后血清中C反应蛋白(CRP)、白细胞介素-1(IL-1)、白细胞介素-6(IL-6)、β-内啡肽(β-EP)含量。结果治疗组与对照组临床疗效总有效率分别为92.68%、85.71%,治疗组优于对照组(P<0.05)。与治疗前比较,两组疼痛评分均显著降低(P<0.05);治疗后两组疼痛评分比较,治疗组均低于对照组(P<0.05)。与本组治疗前比较,两组CRP、IL-1、IL-6均明显降低,β-EP均明显升高(P<0.05)。治疗后治疗组IL-1、IL-6低于对照组,β-EP高于对照组(P<0.05)。结论温针灸配合委中穴拍打放血能更有效改善腰椎间盘突出症患者的疼痛,提高临床疗效。

Objective To observe the clinical curative effect of warm acupuncture and moxibustion combined with beating and bloodletting at acupoint Weizhong（ BL 40） in the treatment of lumbar disc herniation. Methods86 cases of lumbar disc herniation were randomly divided into a treatment group and a control group,with 43 cases in each group. The treatment group was given warm acupuncture and moxibustion combined with beating and bloodletting at Weizhong（ BL 40）,the control group was treated only with warm acupuncture and moxibustion. Both the two groups were treated once daily,7 times for 1 course,with treatment interval of 2 days. The curative effect was figured out after 3 courses of treatment. After treatment,the clinical efficacy and the pain scores were compared between the two groups,and C-reactive protein（ CRP）,white blood cell mediated endothelin 1（ IL-1）,leukocyte mediated IL-6（ IL-6）,and beta endorphins（ EP） content in serum were detected before and after treatment. Results The total effective rate of the treatment group and control group was 92. 68% and 85. 71% respectively,with a significant difference between the two groups（ P 〈 0. 05）. Compared with before treatment,pain scores were significantly lower in both groups（ P 〈 0. 05）,especially in the treatment group（ P 〈 0. 05）. Compared with the condition before treatment,CRP,IL-1 and IL-6 was significantly decreased,andβ-EP of the two groups was significantly increased（ P 〈0. 05） in both groups,especially IL-1,IL-6,andβ-EP in. the treatment group（ P 〈 0. 05）. Conclusion Warm acupuncture and moxibustion combined with beating and bloodletting at Weizhong（ BL 40） can ease the pain of lumbar intervertebral disc herniation more effectively to improve the clinical efficacy.
