ERIC-PCR分析铜绿假单胞菌菌种分型及耐药性特征

Analysis of the Genotyping and Drug Resistance Characteristics of Pseudomonas aeruginosa by ERIC-PCR

摘要 目的:探讨肠杆菌基因间重复一致序列为引物的聚合酶链反应同源性技术(ERIC-PCR)在铜绿假单胞菌(PA)菌种分型与耐药性特征分析中的应用。方法:采用ERIC-PCR快速检测526份临床感染标本中PA各亚型及其耐药基因。结果:共分离出57株PA(10.84%),均分离于下呼吸道感染痰液标本;PA分布以呼吸科为主(49.12%),其余为外科危重症科(35.09%)、神经内科(24.56%)和妇儿科(5.26%),差异具有统计学意义(P<0.05)。ERIC-PCR图谱显示,A、B型PA比例显著高于C、D、E、F、G、H型PA,差异具有统计学意义(P<0.05)。PA对哌拉西林和氨曲南耐药率很高,分别为91.23%和85.96%;而对头孢吡肟耐药率最低,为40.35%,差异具有统计学意义(P<0.05)。PA浓度≥103CFU/ml时,ERIC-PCR对PA和耐药基因阳性检出率为100%,PA浓度为102、10 CFU/ml时,PA阳性检出率分别为80%、70%。结论:该方法可用于PA菌种分型和耐药特征评估,对指导临床抗感染治疗具有重要意义。 OBJECTIVE: To investigate the application of ERIC-PCR homology analysis in genotyping and drug resistance characteristics of Pseudomonas aeruginosa (PA). METHODS: ERIC-PCR was conducted to detect the PA subtypes and drug resistance gene of 526 infectious specimens. RESULTS: 57 PA infectious specimens were separated (10.84%) from sputum specimens of lower respiratory tract infection; PA was mainly from respiration department (49.12%), followed by department of surgical critically care (35.09%), neural internal department (24.56%) and gynecology and pediatrics (5.26%), with significant differences (P〈0-05). ERIC-PCR showed that the proportion of type A and B of PA strains was significantly higher than type C, D, E, F, G and H (P〈 0-05) , with significant differences (P〈0-05). PA showed the high drug resistant rate to piperacillin (91.23%) and aztreonam (85.96%), and the lowest to cefepime (40.35%), with significant differences (P〈0-05). When the PA concentration was no less than 103 CFU/ml, the positive detection rate of PA and resistance gene to ERIC-PCR were 100% ; when it was 102 CFU/ml and 10 CFU/ml, the positive detection rate of PA was respectively 80% and 70%. CONCLUSIONS: The method can be used for the evaluation of the genotyping and drug Resistance of PA and has great significance in the guidance of anti-infectious medication in the clinic.
