社区糖尿病患者系统管理的效果观察

To Observe the Effect of Diabetic Patients in Community Management System

目的探讨针对糖尿病实施社区系统管理的效果及推广价值。方法选取笔者所在镇辖地区90例2型糖尿病患者作为研究对象,分为实施社区系统管理实验组和常规管理的对照组,比较2组糖尿病相关知识知晓率、血糖控制率、医疗费用支出等差异。结果干预后2组患者糖尿病效果知识知晓合格率较干预前均显著上升,不合理显著下降(P<0.01),但系统管理模式干预的实验组改善幅度更明显(P<0.05)。实验组干预后FPG、2h-PG、糖化血红蛋白和血压等四项指标均达到血糖控制目标值,平均医疗费用支出下降幅度明显大于对照组(P<0.05)。结论社区管理系统模式对乡镇卫生机构而言能有效提高糖尿病防控质量,减少医疗费用支出,值得在乡镇一级推广。

Objective To explore the implementation of community management system for diabetes effect and popularization value. Methods The author is located in the town of jurisdiction area of 90 cases of type 2 diabetes mellitus patients as research subjects, divided into the control group the implementation of community management system of experiment group and routine management, compared 2 groups of diabetes related knowledge awareness rate differences, blood glucose control rate, medical expenses etc.. Results After the intervention, the qualified rate increased significantly compared with that before the intervention effect of knowledge awareness 2 groups of diabetic patients, unreasonable decreased significantly （P 〈 0.01）, but the experimental group system management mode of intervention the magnitude of improvement is more obvious （P 〈 0.05）. The experimental group of four indicators of prognosis of FPG, dry 2h-PG, glycosylated hemoglobin and blood pressure reached the goal of blood glucose control value, the average medical expenditure decreased significantly greater than the control group （P 〈 0.05）.Conclusion Community management system can effectively improve the quality of diabetes prevention and control for township health institutions, reduce the medical expenses, it is worth in the township level promotion.
