儿童特发性血小板减少性紫癜血清B淋巴细胞激活因子表达分析

Analysis of B Cell Activating Factor Levels in Children with Idiopathic Thrombocytopenic Purpura

目的分析儿童特发性血小板减少性紫癜血清B淋巴细胞激活因子表达情况,为其诊断及预后提供参考依据。方法以2012年9月-2014年6月本院收治的120例初诊为特发性血小板减少性紫癜患儿及60例同期健康体检的正常儿童为研究对象,比较急性型组和慢性型组及正常儿童组血清B淋巴细胞激活因子表达情况、抗血小板抗体PAIg G、PAIg M和PAIg A水平及血小板计数情况。结果急性型组和慢性型组特发性血小板减少性紫癜患儿外周血B淋巴细胞激活因子、PAIg G、PAIg M和PAIg A含量均高于对照组,但是其血小板计数少于对照组,差异均有统计学意义(P均<0.05);其血清B淋巴细胞激活因子含量与PAIg G、PAIg M和PAIg A含量呈正相关,与血小板计数呈负相关,其疗效与BAFF含量也呈负相关。结论特发性血小板减少性紫癜患儿血清中B淋巴细胞激活因子表达增加,且与抗血小板抗体、血小板计数以及治疗效果具有良好的相关性,其可作为该病诊断的参考依据。

Objective To study B cell activating factor levels and its significance in the diagnosis and prognosis of chil- dren with idiopathic thrombocytopenic purpura（ITP）. Methods Subjects included 120 children with idiopathic thrombocytopenic purpura and 60 normal controls who received physical examination in our hospital from September 2012 to June 2013. B cell activating factor （BCAF） levels, platelet antibodies PAIgG, PAIgM, PAIgA content and platelet count were compared among patients with acute and chronic ITP, and normal controls. Results The levels of BCAF, PAIgG, PAIgM, PAIgA were significantly higher in patients with acute and chronic [TP than in the normal controls（P 〈 0.05）. But platelet count was less in ITP than in the normal control. BCAF levels were positively correlated with PAIgG, PAIgM, PAIgA levels and negatively correlated with platelet count. Therapeutic efficacy for ITP was negatively correlated with BCAF levels. Conclusion BCAF levels increase in idiopathic thrombocytopenic purpura. Since BCAF levels are correlated with antiplatelet antibody, platelet count and therapeutic efficacy, it could be used as reference for the diag- nosis and prognosis of idiopathic thrombocytopenic purpura.
