解毒活血汤联合四妙散加减治疗急性盆腔炎41例

Forty-One Patients with Acute Pelvic Inflammation Treated with Toxicity-Removing and Blood-Activating Decoction Combined with Wonderfully Effective Four Powder

目的:观察解毒活血汤联合四妙散治疗急性盆腔炎的临床疗效。方法:选择2013年2月—2014年11月就诊于本院的82例急性盆腔炎患者,随机分为对照组和观察组各41例。对照组患者给予盐酸左氧氟沙星及奥硝唑静脉滴注治疗,日2次,观察组患者在对照组治疗的基础上给予解毒活血汤联合四妙散加减治疗,日1剂,1周为1个疗程,连续治疗3周。结果:治疗后患者下腹疼痛、发热、白带量多、乏力等临床症状改善明显;对照组有效率为70.7%,观察组有效率为90.2%,观察组的临床症状改善及有效率明显高于对照组,差异有统计学意义(P<0.05);两组患者机体炎性细胞因子于治疗后变化明显,CRP、WBC、IL-6、TNF-α水平较治疗前下降,而IL-2水平则上升,观察组的变化幅度更明显,差异有统计学意义(P<0.05);患者经治疗后机体免疫力有所提高,与免疫力相关的血清免疫球蛋白Ig G、Ig A、Ig M及T淋巴细胞亚群CD3+、CD4+、CD8+水平较治疗前明显升高,以观察组变化更为明显,差异具有统计学意义(P<0.05)。结论:解毒活血汤联合四妙散可以降低炎性因子水平,提高机体免疫力,疗效确切。

Objective： To observe the clinical effect of Toxicity-Removing and Blood-Activating Decoction combined with Wonderfully Effective Four Powder on acute pelvic Inflammation（ API）. Methods： 82 cases with API received by this hospital from February,2013 to December,2014 were randomized into control group（41 cases） given Levofloxacin Hydrochloride and Ornidazole Intravenous Drip treatment with twice every day and treatment group（41 cases） additionally given Toxicity-Removing and BloodActivating Decoction combined with Wonderfully Effective Four Powder with one dosage every day and for 3 treatment courses（one week as a treatment courses）. Results： The clinical symptoms of lower abdominal pain,fever,profuse leukorrhea,and listlessness got obviously improved. The effective rate of observation group（ 90. 2%） was significantly higher than that of control group（70. 7%）; the difference had statistical significance（ P〈0. 05）. The inflammatory cytokines in both groups changes greatly after the treatment： the levels of CRP,WBC,IL-6 and TNF-α declined after the treatment,while the level of IL-2 increased. The changes in observation group were much more obvious,with statistical significance（ P〈0. 05）. The immunity of the patients got enhanced and the levels of serum immunoglobulins（ Ig G,Ig A,Ig M）,and T-lymphocyte subsets（ CD3 ＋,CD4＋,CD8 ＋） increased obviously in both groups and the increase was more obvious in observation group; the difference had statistical significance（ P〈0. 05）. Conclusion： Toxicity-Removing and Blood-Activating Decoction combined with Wonderfully Effective Four Powder can lower the level of inflammatory cytokines and enhance the body immunity with definite efficacy.
