摘要
目的探讨老年心衰患者血浆BNP水平与心功能分级之间的关系,为老年心衰患者的早期诊断与心功能分级提供依据。方法选取2011年1月至2012年2月就诊于乌兰察布中心医院的40例老年心衰患者为观察组,其中NYHA心功能分级Ⅰ-Ⅱ级的20例患者为轻度心衰亚组、NYHA心功能分级Ⅲ-Ⅳ级的20例重度心衰亚组,以同期20例健康体检者为对照组,检测所有观察对象血浆BNP水平。结果对照组血浆BNP水平为(35.52±20.61)pg/ml、轻度心衰亚组血浆BNP水平为(107.03±17.23)pg/ml、重度心衰亚组血浆BNP水平为(744.58±159.04)pg/ml,心衰组血浆BNP水平均明显高于对照组,差异有统计学意义(P=0.047和0.000,均〈0.05)。结论老年心衰患者血浆BNP水平明显增高且与心功能分级有关,检测血浆BNP水平可用于心衰的早期诊断及判断心衰严重程度。
Objective To discuss the correlation between the plasma BNP level and heart function classification in elderly patients with heart failure,providing evidence for the early diagnosis and cardiac function classification of elderly patients with heart failure. Methods 40 cases of elderly patients with heart failure were treated in the Wulanchabu Central Hospital as the study group from January 2011 to February 2012, including 20 cases patients with heart function of NYHA grade Ⅰ-Ⅱ as mild heart failure subgroup and 20 cases patients with heart function of NYHA grade Ⅲ-Ⅳ as severe heart failure subgroup. 20 healthy individuals in the same period were as the control group. The plasma BNP levels of all the observed objects were detected. Results The plasma BNP level in the control group was (35.52± 20. 61) pg/ml,in mild heart failure subgroup was(107.03±17.23) pg/ml,in severe heart failure subgroup was(744.58±159.04) pg/ml. The plasma BNP level in heart failure group was significantly higher than that in the control group,the difference was statistically significant(P=0. 047 and 0. 000,〈 0. 05). Conclusion The plasma BNP level in elderly patients with heart failure increased significantly and was correlation with the classification of cardiac function. The detection of plasma BNP level can be used to early diagnosis and judge the severity of heart failure.
