口腔鳞状细胞癌组织中HHV-6,VEGF-C与CD44表达与颈淋巴结转移

The expression of HHV-6, VEGF-C and CD44 in oral squamous cell carcinoma and its correlation with necklymph node metastasis

摘要 目的研究人类疱疹病毒6型(HHV-6)、VEGF-C与CD44表达与口腔鳞状细胞癌颈部淋巴结转移的相关性及其临床意义。方法运用免疫组化检测40例口腔鳞状细胞癌、21例癌前病变及26例正常对照组组织中HHV-6、VEGF-C与CD44表达情况。结果正常对照组、癌前病变组、口腔鳞状细胞癌组组织中VEGF-C检出率分别为0、85.7%、90%,组间比较具统计学差异(P<0.05)。三组中HHV-6检出率分别为0、85.7%、87.5%,3组间HHV-6检出率逐渐提高趋势,组间比较提示统计学有显著性差异(P<0.005)。CD44在三组中的阳性表达率分别是100%、57.1%、12.5%,随着口腔粘膜上皮异型增生和癌前病变至鳞状细胞癌呈现表达逐渐下降之势,组间比较提示统计学有显著性差异(P<0.005)。同时研究表明,口腔鳞状细胞癌组VEGF-C和HHV-6表达与颈淋巴结转移有相关性,而CD44表达与口腔鳞状细胞癌颈淋巴结转移无相关性。结论 HHV-6感染、VEGF-C过度表达和CD44的表达下降可能参与了口腔粘膜上皮细胞异型增生并癌变的过程,与口腔鳞状细胞癌的发生存在相关性。HHV-6感染和VEGF-C过度表达与口腔鳞状细胞癌颈淋巴结转移有相关性。 Objective To study the Clinical significance and the correlation between the expression of human herpes virus type 6 (HHV-6), VEGF-C and CD44 in squamous cell carcinoma tissue and cervical lymph node metastasis. Methods The immunohistochemistry was performed to detect the expression of HHV-6, VEGF-C and CD44 in 40 cases with oral squamous cell carcinoma (OSCC group), 21 cases with precancerous lesions (pre-eancer group) and 26 cases with normal oral mucosa (control group). Results The expression of VEGF-C in control group, pre-cancer group and OSCC group was 0, 85.7%, 90% , respectively and there was significant difference between pre-cancer group and control group (P〈0.05), OSCC group and control group (P〈0.05). The positive expression rate of HHV-6 in control group, pre-eancer group and OSCC group was 0, 85.7%, 87.5%, respectively and there was significant difference between pre- cancer group and control group (P〈0.005), OSCC group and control group (/9〈0.005). The rate of positive expression of CD44 in control group, pre-cancer group and OSCC group was 100%, 57.1%, 12.5% , respectively and there was significant difference between pre-caneer group and control group (P〈0.005), OSCC group and control group (/9〈 0.05). The expression of VEGF-C and HHV-6 in oral squamous cell carcinoma was related to the cervical lymph node metastasis while CD44 expression had no correlation with the lymph node metastasis. Conclusion HHV-6 infection, VEGF-C over-expression and the down-regulated expression in CD44 might participate in the process of oral mucosal epithelial cell hyperplasia and carcinogenesis. The expression of HHV-6, VEGF-C and CD44 was related to the occurrence and existence of oral squamous cell carcinoma. There was correlation between the HHV-6 infection, VEGF-C over-expression and oral squamous cell carcinoma with neck lymph node metastasis.
