子宫内膜癌细胞学筛查的Meta分析

A meta analysis on cytologic screening application in diagnosis of endometrial carcinoma

摘要 目的 探讨宫腔细胞学检查在子宫内膜癌筛查的诊断价值。方法 计算机检索1970~2014年国内外公开发布的以子宫内膜病理检查为金标准的关于宫腔细胞学筛查的中英文文献,提取相关数据。根据Cochrane协作网推荐的方法对文献进行质量评价,并用Stata 12.0软件进行统计分析。结果 共纳入文献15篇,子宫内膜癌细胞学筛查的合并敏感度和特异度分别为0.86、1.00,诊断比值比为1 241.00,合并阳性和阴性似然比分别为175、0.14,总受试者工作特征曲线下面积为0.99。结论 宫腔细胞学检查用于筛查子宫内膜癌具有可行性。 Objective To access the diagnosis value of cytologic screening in the identification of endometrial carcinoma. Methods To retrieve the relevant literature published on both Chinese and international journals covering January 1970 to December 2014 on cytologic screening in endometrial carcinoma and then extract the related data from the literature.The academic quality of retrieved papers were evaluated by a methodology recommended by Cochrane Network, and meta-analysis was performed by Stata12.0 software.Results Fifteen papers were accepted; the pooled sensitivity value of cytologic screening for the diagnostic of endometrial carcinoma was 0.86, the specificity value was 1.00, the odd ration was 1 241.00, while the pooled positive and negative likelihood ratio were 175 and 0.41 respectively;the area under the curve (AUC) of the total receiver operating indicatrix was 0.99.Conclusion Cytologic screening is a feasible diagnostic method for endometrial carcinoma.
