超声引导下经皮胆囊穿刺置管引流术在胆道炎症治疗中的应用

Percutaneous gallbladder catheterized drainage under ultrasound guidance in treating inflammation of gallbladder and biliary ducts

目的:探讨超声引导下经皮胆囊穿刺置管引流术(PGCD)在胆道炎症治疗中的应用。方法:急性胆囊炎、胆管炎症患者64例,采用PGCD治疗,观察患者术前、术后生命体征、每日引流量、实验室检查。结果:所有病例均1次穿刺成功,患者临床症状缓解迅速,术后患者体温、白细胞(尤其是中性粒细胞)、黄疸指数下降,未发生大出血、胆漏等并发症。结论:PGCD治疗急性胆囊炎及胆管炎症的成功率高、疗效好、并发症少。

Objectlve： To explore the clinical value of ultrasound guided percutaneous gallbladder catheterized drainage（PGCD）in the treatment of inflammation of gallbladder and biliary ducts. Methods： 64 patients with acute cholecystitis or bile duet inflammation were treated by PGCD, then vital sings,daily bile flow were observed before and after treatment, along with the laboratory examination. Results： All patients were successful in first PGCD, with quick remission of clinical symptoms. Meanwhile, their body temperature, white blood cells （especially neutrophils）, jaundice index decreased after drainage. Bleeding, bile leakage and other complications clid not occur. Conclusion：PGCD has higher success rate, good efficacy and fewer complications in treatment of acute cholecystitis and bile duct inflammation.
