Bakri球囊填塞术治疗难治性产后出血12例临床分析

The Clinical Analysis of the Application of Bakri Balloon tamponade in the Treatment of 12 Cases of Post-partum Haemorrhage

目的评价Bakri球囊填塞术治疗难治性产后出血的临床疗效。方法回顾性分析2013年6月至2014年6月本院难治性产后出血并使用Bakfi球囊治疗的产妇12例,总结分析病例特点及止血治疗效果。结果单纯应用Bakri球囊填塞止血成功8例,应用Bakri球囊填塞同时行子宫动脉下行支结扎术止血成功3例,放置球囊后失败而行子宫切除术1例。11例止血治疗成功并保留子宫的产妇,术后无发热、子宫内膜炎、切口感染、子宫切口愈合不良以及子宫坏死等并发症发生,产后6 w随访,无晚期产后岀血发生。结论 Bakri球囊填塞术作为产后出血的保守治疗方法,其操作简单、止血疗效肯定、无术后并发症,具有临床推广应用价值。

Objective To evaluate the clinical effectiveness of intrauterine Bakri balloon tamponade in the treatment of intractable post - partum haemorrhage. Methods A retrospective study of 12 patients who underwent Bakri balloon insertion after unsuccessful medical management of post - partum haemorrhage. The clinical characteristics and hemostatic effects were summarized and analyzed. Results Intrauterine Bakri balloon tamponade was successfully used to stop the post -partum haemorrhage in 8 cases. Bakri balloon tamponade in combination with uterine artery descending branch ligation was used successfully to stop the post - partum haemorrhage in 3 cases. Hysterectomy was performed in 1 case after the failure of Bakri balloon tamponade. Among the 11 cases of lying - in woman who were cured successfully and retained the womb, no fever, endometritis, incision infection, poor healing of uterus incision and uterine necrosis or other complications occurred after operation. And no more post - partum haemorrhage occurred in 6 weeks. Conclusion Bakri balloon tamponade, as a conservative procedure in the treatment of post - partum haemorrhage, proves to be simple, effective and safe, worth of clinical popularization and application.
