杨氏萆薢分清饮和桂枝加葛根汤加减治疗寒湿型腰椎间盘突出症临床观察

Clinical Observation on the Treatment of Cold-dampness-type Lumbar Disc Herniation with Yang's Bixie Fenqing Drink(杨氏萆薢分清饮)and Guizhi Jia Gegen Decoction(桂枝加葛根汤)

目的:观察杨氏萆薢分清饮和桂枝加葛根汤加减治疗寒湿型腰椎间盘突出症的临床疗效。方法:将90例寒湿型腰椎间盘突出症患者随机分为治疗组和对照组,每组45例。治疗组给予杨氏萆薢分清饮和桂枝加葛根汤加减治疗,对照组给予双氯芬酸钠缓释片口服治疗。两组均以20 d为1个疗程,1个疗程及随访6个月后,评定患者临床疗效、安全性指标及远期疗效,并进行统计学分析。结果:1个疗程后,治疗组显效30例,有效12例,无效3例,总有效率为93.33%;对照组显效28例,有效13例,无效4例,总有效率为91.11%。两组比较,差异无统计学意义(P>0.05)。治疗组无不良反应发生;对照组发生不良反应7例,其中胃肠道不适5例,肝功能损害1例,皮疹1例,停药后自行好转。两组不良反应比较,差异有统计学意义(P<0.05),治疗组较对照组安全性高。随访6个月,治疗组显效29例,有效11例,无效5例,总有效率为88.89%;对照组显效22例,有效9例,无效14例,总有效率为68.89%。两组比较,差异有统计学意义(P<0.05)。结论:采用杨氏萆薢分清饮和桂枝加葛根汤加减治疗寒湿型腰椎间盘突出症,临床疗效满意,安全性较好,且远期疗效可观,值得在临床推广应用。

Objective：To observe the clinical curative effect of Yang＇s Bixie Fenqing Drink（杨氏萆薢分清饮）and Guizhi Jia Gegen Decoction（桂枝加葛根汤）in the treatment of cold-dampness-type lumbar disc herniation.Methods：90 cases of cold-dampness-type lumbar disc herniation were randomly divided into a treatment group and a control group,45 cases in each group.The treatment group were treated with Yang＇s Bixie Fenqing Drink and Guizhi Jia Gegen Decoction,while the control group were treated with diclofenac sodium sustained-release tablets.20 days was a course of treatment.After 1 course and 6-month follow-up,the clinical efficacy,safety index and the long-term curative effect were evaluated,and the statistical analysis was carried out.Results：After 1 course,in the treatment group,30 cases were excellent,12 cases were effective,and 3 cases were ineffective,the total effective rate being 93.33%;in the control group,28 cases were excellent,13 cases were effective,and 4 cases were ineffective,the total effective rate being 91.11%.There was no significant difference between the two groups（P〉0.05）.There were no adverse reactions in the treatment group,while there were 7 cases in the control group having adverse reaction,5 cases with gastrointestinal discomfort,1 case with liver function damage,and 1 case with rash,getting better after stopping the drug.The differences of adverse reactions inthe two groups were statistically significant（P〈0.05）,and the treatment group was more safe than the control group.After6-month follow up,in the treatment group,29 cases were markedly effective,11 cases were effective,and 5 cases were invalid,the total effective rate being 88.89%;while in the control group,22 cases were markedly effective,9 cases were effective,and 14 cases were invalid,the total effective rate being 68.89%.The difference between the two groups was statistically significant（P〈0.05）.Conclusion：Yang＇s Bixie Fenqing Drink and Guizhi Jia Gegen Decoction are effective and safe in the treatment of cold-dampness-type lumbar disc herniation,worthy of being clinically applied.
