摘要
目的观察复方苦参注射液联合拉米夫定治疗慢性乙型肝炎的临床疗效。方法将140例慢性乙型肝炎患者随机分为治疗组(70例)和对照组(70例),对照组在常规保肝、对症、支持基础治疗上,口服拉米夫定100mg,每日1次。治疗组在对照组治疗基础上,用复方苦参注射液20ml加入250ml 0.9%氯化钠注射液中,静脉滴注,每日1次,2组疗程均为6周,比较2组患者治疗前后肝功能及HBV DNA定量的变化。结果治疗6周后,对照组ALT、GGT水平显著低于治疗前(P<0.05),治疗组ALT、AST、TBil、GGT水平显著低于治疗前(P<0.05),且与对照组比较,治疗组ALT、AST、TBil、ALP、GGT水平显著低于对照组(P<0.05,P<0.01),治疗组HBV DNA转阴率(80.54%)显著高于对照组(56.90%)(P<0.05),治疗结束后随访6个月,治疗组复发率(10.00%)显著低于对照组(17.50%)(P<0.05)。结论复方苦参注射液联合拉米夫定治疗慢性乙型肝炎可明显改善肝功能,促进HBV DNA阴转,其作用优于单用拉米夫定。
Objective To observe the clinical effect of compound Kushen injection combined with lamivadine in treating chronic hepatitis B. Methods 140 patients with chronic hepatitis B were divided into treatment group(70 cases) and control group (70 cases) randomly. The patients of control group were treated by lamivadine,100 mg/ time and ltimes/d orally. The patients of treatment group were treated by compound Kushen injection 20 ml/time and 1 times/d I. v. drip on the basis of treatment of the control group. The therapy course lasted for 6 weeks. The indexes of liver function and HBV DNA in both groups were compared. Results After the treatment for 6 weeks, the level of ALT,GGT in the control group and the level of ALT,AST,TBil,GGT in the treatment group were ob- viously lower than those before treatment (P〈0.05). The level of ALT,AST,TBil,ALP,GGT in the treatment group were obviously lower than those of the control group (P〈0.05 ,P〈0.01). The negative conversion rate of HBV DNA in the treatment group was obviously higher than that of the control group (P〈0.05). The reoccur- rence rate in the treatment group was obviously lower than that of the control group (P〈0.05). Conclusion Compound Kushen injection combined with lamivadinec could improve the liver function and promote HBV DNA negative conversion in treating chronic hepatitis B. The clinical effect of compound Kushen injection combined with lamivadine in treating chronic hepatitis B was supeiior to that of lamivadine.
