摘要
目的探讨红细胞、血小板参数在新生儿缺氧缺血性脑病(HIE)中的变化及其意义。方法利用Sysmex XE-5000全自动血细胞分析仪对29例HIE患儿和30例正常新生儿进行红细胞及血小板等参数检测,分析其结果和临床意义。结果 1HIE组与对照组红细胞参数结果比较发现:红细胞压积(HCT)、红细胞平均体积(MCV)、平均血红蛋白浓度(MCHC)、红细胞分布宽度CV(RDW-CV)、红细胞分布宽度SD(RDW-SD)、网织红细胞计数(RET#)、网织红细胞百分比(RET%)、未成熟网织红细胞(IRF)、低荧光网织红细胞比率(LFR)、高荧光网织红细胞比率(HFR)、有核红细胞计数(NRBC#)和有核红细胞百分比(NRBC%)等12项红细胞参数差异有统计学意义,而红细胞计数(RBC)、血红蛋白(HGB)、平均血红蛋白含量(MCH)和中荧光网织红细胞比率(MFR)等参数差异无统计学意义;2HIE组与对照组血小板参数结果比较发现:血小板计数(PLT)和血小板压积(PCT)差异有统计学意义,而平均血小板体积(MPV)、血小板分布宽度(PDW)及大型血小板比率(P-LCR)等参数差异无统计学意义。结论利用Sysmex XE-5000全自动血细胞分析仪对HIE患儿外周血进行红细胞及血小板参数检测,计数快速、准确,实验结果特异、灵敏,可以作为协助新生儿缺氧缺血性脑病诊断的有效指标,满足临床需要。
[ Objective ] To discuss the changes of red blood cell and platelet parameter in neonatal hypoxic-is- chemic encephalopathy (HIE) and its significance. [Methods] The Sysmex XE-5000 automatic blood cell analyzer was used to analyze the red blood cells and platelet in 29 HIE children patients and 30 healthy babies. The results and clinical implications were analyzed. [ Results]①There were significant differences in 12 blood cell parameters between the HIE group and control group: hematocrit (HCT), mean corpusclar volume (MCV), mean corpuscular hemoglobin concentration (MCHC), red blood cell distribution width-coefficient of variation (RDW-CV), red blood cell distribution width-standard deviation (RDW-SD), reticulocyte count (RET#), reticulocyte percentage (RET%), immature reticulocyte fraction (IRF), low fluorescent ratio (LFR), high fluorescent ratio (HFR), nucleated red blood cell count (NRBC#) and nucleated red blood cell percentage (NRBC%). However, there was no significant difference in red blood cell count (RBC), hemoglobin (HGB), mean corpuscular hemoglobin (MCH), middle fluorescent ratio (MFR); ② The comparison of platelet parameters between the HIE group and the control group showed significant differences in platelet count (PLT) and plateletcrit (PCT). However, there was no significant difference in mean platelet volume (MPV), platelet distribution width (PDW), platelet-large cell ratio (P-LCR) and other parameters be- tween the two groups. [ Conclusions ] Sysmex XE-5000 automatic blood cell analyzer is fast and accurate in examin- ing the red blood cell and platelet parameters of peripheral blood of children patients with hypoxic ischemic en- cephalopathy. The experimental results are specific and sensitive. It can provide effective indicators to assist the di- agnosis of neonatal hypoxic ischemic encephalopathy, and meet clinical needs.
