抗癌消水膏外敷联合顺铂胸腔内灌注治疗癌性胸腔积液临床研究

Clinical Investigation on Kangai Xiaoshui Ointment Applied Externally Combined with Intrapleural Perfusion of Cisplatin in Treatment of Cancerous Pleural Effusion

目的探讨抗癌消水膏外敷联合顺铂注射液胸腔内灌注对癌性胸腔积液的临床疗效及患者生活质量的影响。方法选取本院2012-08～2014-09呼吸内科收住入院的恶性肿瘤并发胸腔积液患者90例,将上述患者随机分为对照组、治疗组2组,每组各45例。对照组患者予肿瘤常规治疗,并予胸腔穿刺闭式引流,治疗组是在对照组治疗基础上,另予抗癌消水膏外敷联合顺铂注射液胸腔内灌注,观察2组患者胸腔积液消长情况及前后的生活质量KPS评分。结果治疗后,对照组胸腔积液治疗的有效率为57.8%,治疗组胸腔积液治疗的有效率为80.0%,差异具有统计学意义（P=0.023〈0.05）;对照组生活质量KPS评分总有效率71.1%,治疗组生活质量KPS评分总有效率88.9%,差异具有统计学意义（P=0.035〈0.05）。结论抗癌消水膏外敷联合顺铂注射液胸腔内灌注疗效确切,适用于癌性胸腔积液的治疗,是抑制恶性胸腔积液的有效方法之一。

OBJECTIVE To investigate the clinical efficacy and quality of life for patients with cancerous pleural effusion treated by Kangai Xiaoshui ointment applied externally in eombination with infusing eisplatin intrathoracic. METHODS A total of 90 cases of malignant pleural effusion which were selected from cases in Respiratory Medicine between August 2012 to September 2014, were randomly divided into control group （n = 45 ） and the treatment group （n = 45 ）. Patients in the control group were treated with conventional therapy and the thoracentesis closed drainage. Patients in the treatment group were treated with the Kangai Xiaoshui ointment applied externally in combination with infusing cisplatin intrathoracic, on the basis of the treatment of the control group. The amount of the pleural effusion and the quality of life （ KPS score ） of patients in the 2 groups were observed before and after the treatment. RESULTS The effeetive rate in pleural effusion of the control group was 57.8%, while that of the treatment group was 80.0%, and the difference was statistically significant （P= 0.023〈0.05 ）. The total effective rate of the quality of life （KPS scores） of the control group was 71.1%, while that of the treatment group was 88.9%, and the difference was statistically significant （P = 0.035〈 0.05）. CONCLUSION Kangai Xiaoshui ointment applied externally in combination with infusing cisplatin intrathoracic is effective. It is suitable for the treatment of malignant pleural effusion and also is one of the effective methods to inhibit malignant pleural effusion.
