超声内镜对上消化道黏膜下病变的诊断及治疗价值

Value of EUS in the Diagnosis and Treatment of SML

目的:评估超声内镜(EUS)在上消化道黏膜下病变(SML)诊断及治疗中的应用价值。方法:选取77例SML者进行EUS检查,部分患者在确诊后行超声内镜治疗,分析EUS诊断及治疗结果。结果:EUS共检出79处病灶,食管12处(15.19%)、胃63处(79.75%)、十二指肠4处(5.06%)。病理诊断准确率为68.35%,其中囊肿的诊断准确率为100%,间质瘤为94.44%,脂肪瘤为80.00%,平滑肌瘤为57.89%,异位胰腺为50.00%,但未检出神经鞘瘤、类癌、增生性息肉。结论:EUS检查对SML病变性质诊断具有较高的准确率,并且在EUS指导下治疗SML具有良好的安全性和有效性。

Objective： To assess the value of EUS in the diagnosis and treatment of SML.Methods：77 cases of SML patients with EUS examination,some patients treated by EUS therapeutic after the diagnosis,analysis EUS diagnosis and treatment results. Results：EUS examination detected 79 lesions,12 lesions （15.19%） on esophagus,63 lesions （79.75%） on stomach,and 4 lesions （5.06%） on duodenum. Pathological diagnostic accuracy was 68.35%,which diagnostic accuracy was 100% cysts,stromal tumor was 94.44%,80.00% lipomas,leiomyomas was 57.89 percent,50.00 percent of ectopic pancreas,but not detected in nerve sheath tumors,carcinoid tumors,hyperplastic polyps.Conclusion：EUS examination of the SML lesions with high diagnostic accuracy,and under EUS guidance on the treatment of gastrointestinal mucosal lesions with good safety and efficacy.
