67例高度近视患者小切口白内障囊外摘除人工晶状体植入术疗效

Clinical analysis of 67 cases of small incision cataract extracapsular extraction intraocular lens implantation

目的研究分析67例小切口白内障囊外摘除人工晶状体植入术疗效。方法从2011年4月到2014年11月,选取我院的134例眼白内障合并高度近视患者进行回顾性分析,将其分为两组,其中观察组67例患者进行小切口白内障囊外摘除联合人工晶体植入术,对照组67例患者给予传统白内障囊外摘除加入人工晶体植入术治疗,分析对比两组患者的治疗疗效以及并发症情况。结果观察组脱盲率(视力≥0.05)和脱残率(视力≥0.3)占比97.01%(65/67)、92.54%(62/67),均高于对照组的94.03%(63/67)、89.55%(60/67),差异均无统计学意义(均P>0.05);两组患者术后视力比较,差异无统计学意义(P>0.05);观察组高眼压、悬韧带断裂、玻璃体脱出、角膜内皮水肿等并发症发生率均较对照组显著降低(P<0.05)。结论小切口白内障囊外摘除人工晶状体植入治疗白内障高度近视术后恢复快,手术并发症少,值得临床进一步推广。

Objective To study the clinical effects of 67 cases of small incision cataract extracapsular extraction intraocular lens implantation（IOL）. Methods From April 2011 to November 2014, 134 cases of cataracts and high myopia were retrospectively analyzed, and they were divided into two groups. The observation group, 67 patients underwent with small incision cataract extracapsular extraction combined iol implantation and the control group, 67 patients underwent traditional extracapsular extraction and iol implantation. The treatment efficacy and complications of two groups were analyzed. Results The rate of better than 0.05 and 0.3 in observation group accounted for 92.54%（62/67） and 97.01%（65/67）, which are higher than the control group＇s 89.55%（60/67） and 94.03%（63/67）. There was no statistically significant difference（P 〉0.05）; The incidence of complications in observation group, such as high intraocular pressure, suspensory ligament rupture, vitreous prolapsed and corneal endothelial edema was significantly less than the control group（P〈 0.05）. Conclusions Small incision extracapsular cataract extraction and intraocular lens implantation for patients of cataract and high myopia recover fast and had less postoperative complications. It is worth further promotion in the clinical practice.
