摘要
目的评价多排螺旋CT三期增强扫描与MRI平扫对肝脓肿炎症期的诊断价值。方法择肝脓肿炎症期患者51例为研究对象,先后行多排螺旋CT三期增强扫描及MRI平扫,综合评价两者诊断可靠性。结果胆管源性病变(n=23)行MRI诊断检出例数更高(P<0.05),其MRI表现为环形高信号,CT增强扫描表现为环形强化征象,但延迟期为等密度;门静脉源性病变(n=16)行MRI诊断检出例数更高(P<0.05),其MRI表现为晕征,CT表现为低密度环征,增强扫描下密度由外向内逐渐增加;肝动脉源性病变(n=9)行两种诊断方案,检出例数无显著差异(P>0.05),表现为稍低密度或片状T1、T2信号;临近组织感染(n=3)表现为局灶性片影。结论肝脓肿炎症期CT及MRI诊断均有一定可靠性,合理结合两种诊断方案可能有助于提升诊断效果。
[ Objective ] The diagnostic value of tri-phase Enhancement scan of multi-rank spiral computed tomography and MRI plain scanon the inflammation period of hepatophyma liver abscess can be evaluated. [ Methods ] 51 patients who are in the inflammation period of hepatophyma liver abscess can be selected as research objects, the tri-phase Enhancement scan of multi-rank spiral computed tomography and MRI plain scan can be conducted successively, and the diagnosisreliability of the two methods can be evaluated comprehensively. [ Results ] The diagnosisdetection rate of bile vessel source lesion (n=23) row MRI is higher (P〈0.05), the MRI is annular high signals, the annular intensifying symptom can be reflected by CT enhancement scan, and the lag phase is equal-density. Thediagnosisdetection rate ofportal veinsource lesion (n=16) row MRI is much higher (P〈0.05), the MRI is motion sickness, low-density annular symptom can be reflected by CT, and the density is gradually increased outside in under the enhancement scan. Arteria hepaticasource disease (n=9) changes (P〉0.05), the two diagnostic schemes, the result shows that numerous cases have no significant difference the lower-density or flake-shaped T1 and T2 signals can be reflected. The close tissue infection (n=3) indicate focal slice shadow. [ Conclusion ] CT diagnosis and MRI diagnosis in the inflammationperiod of hepatophyma liver abscess have certain reliability, and the diagnosis effect can be improved by combining the two diagnostic schemes reasonably.
