超声评价冠心病心外膜脂肪层厚度与颈动脉弹性的关系研究 被引量:1

摘要 目的探讨超声评估冠心病心外膜脂肪层厚度(EAT)与颈动脉弹性的相关性研究。方法选择我院2013年7月-2014年7月冠心病患者70例为研究组,选择同期体检健康者70例为对照组,超声检测两组的EAT及颈动脉内中膜厚度(IMT),比较两组的颈动脉IMT及EAT;比较不同病变程度的IMT及EAT值;评估冠心病颈动脉弹性与EAT的相关影响因素。结果研究组的颈动脉IMT、EAT、低密度脂蛋白、腰围、舒张压、体质量指数等指标显著高于对照组,差异有统计学意义(P<0.05);冠心病病变严重程度与患者的颈动脉IMT、EAT、低密度脂蛋白、腰围、舒张压、体质量指数等指标密切相关;重度冠心病病变患者的颈动脉IMT、EAT指标显著高于中轻度冠心病,差异有统计学意义(P<0.05),颈动脉IMT及EAT随着冠心病病变程度加重而逐渐升高。结论超声检测颈动脉IMT及EAT是评估冠脉病变的敏感指标,冠心病病变程度与IMT、EAT、舒张压、低密度脂蛋白胆固醇、腰围、体质量指数等因素密切相关。 [Objective]To probe into the correlation study of CHD(coronary heart disease) epicerdium adipose layer thickness and carotid artery elasticity through ultrasonic evaluation. [Methods]70 CHD patients in our hospital from July 2013 to July 2014 were selected as the experimental group, and another 70 healthy people who had passed physical examination were selected as the control group. The epicardium adipose layer thickness(EAT) and intima-media thickness(IMT) of the two groups were measured by ultrasonic inspection, the carotid artery IMT and EAT of the two groups were compared, the carotid artery IMT and EAT values at different lesion degrees were also compared, and the related influencing factors of CHD carotid artery elasticity and ETA were evaluated. [Results]The carotid artery IMT, ETA, low density lipoprotein, waistline, diastolic pressure and body mass index of the experimental group were significantly higher than those of the control group. The differences were of statistical significance(P〈0.05). The lesion degree of CHD was closely related with the carotid artery IMT, ETA, low density lipoprotein, waistline diastolic pressure and body mass index of the patients. The indexes of carotid artery IMT and EAT of severe CHD patients were significantly higher than those of mild CHD patients. The differences were of statistical significance(P〈0.05). The IMT and EAT were gradually increasing along with the severity of CHD lesion degree. [Conclusion]Ultrasonic inspection of carotid artery IMT and EAT is a sensitive index to evaluate coronary artery lesions. The lesion degree of CHD has a close relationship with IMT, ETA, diastolic pressure, low density lipoprotein cholesterin, waistline, body mass index, etc.
