鼻内镜下同步完成腺样体切除及鼓膜置管术治疗小儿分泌性中耳炎临床分析

Clinical analysis of nasal endoscopic surgery in the treatment of complete synchronization tube in children with otitis media with adenoidectomy and grommet insertion

目的探究鼻内镜下同步完成腺样体切除及鼓膜置管术治疗小儿分泌性中耳炎的临床疗效。方法选取我院于2013年12月-2014年12月收治的小儿分泌性中耳炎患儿87例,随机分为对照组43例和观察组44例,对照组采用鼓膜置管术治疗,观察组采用鼻内镜下腺样体切除联合鼓膜置管术治疗,治疗后观察比较两组患儿临床疗效、中耳积液时间和术后感染情况。结果观察组患儿治疗有效率为97.7%,对照组患儿治疗有效率为88.4%,观察组患儿治疗有效率高于对照组(P<0.05),差异有统计学意义;观察组患儿中耳积液时间明显短于对照组(P<0.05),差异有统计学意义;观察组患儿术后感染率为4.5%,对照组患儿术后感染率为14.0%,两组患儿术后感染率比较差异有统计学意义(P<0.05)。结论采用鼻内镜下同步完成腺样体切除及鼓膜置管术治疗小儿分泌性中耳炎,能够有效缩短患儿中耳炎积液时间,患儿治疗有效率高,术后感染率低,治疗效果显著,值得临床推广。

[Objective]To explore the synchronization is complete endoscopic adenoidectomy and tympanostomy tube, the clinical curative effect of treating pediatric secretory otitis media. Selection methods： our hospital in December 2013 to December 2014, 87 patients with pediatric secretory otitis media, 44 cases of 43 cases were randomly divided into control group and observation group, control group adopted the treatment of tympanostomy tube, observation group with adenoidectomy under endoscope for the treatment of joint tympanostomy tube, to observe the clinical curative effect, comparing two groups of patients after treatment time and postoperative infection of the middle ear effusion. [Results]the observation group of patients treatment effective rate was 97.7%, control group patients treatment effective rate was 88.4%, the observation group of patients treatment efficiency is higher than the control group（P〈0.05）, the difference was statistically significant; Observation group of patients with middle ear effusion time significantly shorter than the control group（P〈0.05）, the difference was statistically significant; Observation group of patients with postoperative infection rate was 4.5%, control group patients with postoperative infection rate was 14.0%, two groups of patients with postoperative infection rates compare the difference was statistically significant（P〈0.05）. [Conclusion]USES the synchronization is complete endoscopic adenoidectomy and tympanostomy tube in the treatment of pediatric secretory otitis media, otitis media effusion patients can effectively shorten the time and treatment in patients with high efficient, low postoperative infection, treatment effect is significant, worthy of clinical promotion.
