患儿肺炎支原体感染的临床表现与治疗研究

Clinical manifestations and treatment of Mycoplasma pneumoniae infections in children

目的探讨患儿肺炎支原体感染的临床诊断以及治疗方法,为患儿肺炎支原体感染的诊断和治疗提供依据。方法回顾性分析2011年5月-2013年5月医院收治的肺炎支原体感染患儿207例,所有患儿均采用酶联免疫法测定血清特异性肺炎支原体IgM抗体呈阳性,经临床诊断为肺炎支原体感染,分析患儿临床表现及治疗转归。结果 207例肺炎支原体感染患儿均有呼吸系统感染临床表现,发生率为39.61%;29例心血管系统、10例泌尿系统、神经系统受累、21例神经系统及8例皮肤黏膜受累患儿经阿奇霉素及对症治疗均治愈;9例血液系统受累患儿给予阿奇霉素、升白细胞及糖皮质激素治疗,8例治愈,1例因病情加重抢救无效死亡。结论肺炎支原体感染患儿的临床表现多种多样,可导致患儿病情复杂化,阿奇霉素以及红霉素大环内酯类抗菌药物能够获得较好的治疗效果,且患儿预后较好。

OBJECTIVE To investigate the clinical diagnosis and treatment of Mycoplasma pneumoniae infections in children ,so as to provide the basis for diagnosis and treatment of M . pneumoniae infections in children . METHODS A retrospective analysis was conducted on 207 children with M .pneumoniae infections admitted to hospital in May 2011 - May 2013 ,all patients were determined as positive for serum M .pneumoniae specific IgM antibody by enzyme linked immunosorbent assay and were confirmed to have M .pneumoniae infection by clinical diagnosis .The clinical presentation and the treatment outcomes were analyzed .RESULTS All the 207 children had clinical manifestation of respiratory system infections with the incidence rate of 39 .61% .The infection involved 29 cases of cardiovascular system ,10 cases of urinary and nervous systems ,21 cases of nervous system and 8 cases of skin membrane and all children in these cases were cured by azithromycin and symptomatic treatment .Children in 9 cases of hematological system involvement were treated with azithromycin ,glucocorticoids and leukocyte‐in‐creasing therapy .Of the 9 cases ,8 were cured and 1 was dead after rescue due to aggravation .CONCLUSION M . pneumoniae infection in children has diverse clinical manifestations which can lead to complication of the disease . Macrolides such as azithromycin and erythromycin can achieve better efficacy and prognosis .
